摘要
目的:探讨药物基因检测在个体化给药、安全用药及指导合理用药中的作用和意义。方法:收集本院2019年5~8月的药物基因检测报告,对检测结果进行统计分析。结果:通过药物基因检测后,在阿司匹林使用患者中,21例(12.14%)更换了治疗方案,3例(1.96%)需要密切监测药物不良反应;在氯吡格雷使用患者中,95例(58.28%)需要进行剂量调整及药物更换;在华法林使用患者中,2例(18.18%)进行了剂量调整;在他汀类药物使用患者中,5例(4.88%)进行了剂量限制和药物不良反应监测;在卡马西平使用患者中,3例(37.5%)需要警示严重不良反应发生并更换治疗药物。结论:通过药物基因检测开展必要的药学服务,予以指导临床用药,并促进个体化用药。
Objective:The purposes are to explore the role of pharmacogenetic testing in individual drug administration,safe use of drug and guidance of clinical rational drug use.Methods:We collected pharmacogenetic testing reports participated by clinical pharmacists from May 2019 to August 2019 and analyzed the testing results.There were 429 cases of pharmacogenetic testing,mainly involved 173 cases of aspirin(40.33%),163 cases of clopidogrel(38.00%),11 cases of warfarin(2.56%),41 cases of statins(9.56%)and 8 cases of carbamazepine.All the cases covered 6 clinical departments of the hospital.Results:Based on the pharmacogenetic testing results,21 cases(12.14%)of aspirin needed a change of treatment regimen,3 cases of aspirin needed close monitoring on adverse drug reactions;95 cases(58.28%)of clopidogrel needed dose adjustment and drug replacement;2 cases(18.18%)of warfarin needed dose adjustment;5 cases(4.88%)of statin needed dose limitation and adverse drug reaction monitoring;3 cases of carbamazepine needed pharmacovigilance of serious adverse reactions and change of therapeutic drugs.Conclusion:The work of pharmacogenetic testing in our hospital had a significant effect.It is necessary to carry out pharmaceutical care with pharmacogenetic testing.Pharmacogenetic testing can guide the use of clinical drug and promote individual drug use.
