福多司坦与消坦立对COPD合并PF患者肺功能及肺纤维化的影响 被引量:4

Effects of Fudosteine and Acetylcysteine Granules on Pulmonary Function and Pulmonary Fibrosis in the Treatment of COPD with Pulmonary Fibrosis

摘要 目的比较福多司坦与消坦立治疗慢性阻塞性肺疾病(COPD)合并肺纤维化(PF)的应用效果.方法选取96例COPD合并PF患者,随机数字表法分为观察组、对照组,每组48例;在常规治疗基础上,观察组给予福多司坦口服,对照组给予消坦立口服,连续治疗6个月,比较两组肺功能[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、最大呼气峰流速(PEF)]、肺纤维化程度[临床、影像、生理(CRP)评分]及相关血清指标[层黏连蛋白(LN)、透明质酸(HA)、Ⅲ型前胶原(PC-Ⅲ)]、氧化应激[丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)]、生活质量[圣乔治呼吸疾病问卷(SGRQ)]及药物副作用.结果治疗后,两组肺功能指标FEV1、FVC、PEF水平均高于治疗前(P<0.05);CRP评分系统中临床症状、影像表现、生理状态各维度评分及总分均低于治疗前(P<0.05);血清LN、HA及PC-Ⅲ水平均低于治疗前(P<0.05);血清MDA水平均低于治疗前(P<0.05),SOD及GSH-Px水平均高于治疗前(P<0.05);SGRQ评分中呼吸症状、疾病影响、活动能力各维度评分均低于治疗前(P<0.05),且观察组上述指标升高或降低幅度大于对照组(P<0.05);两组药物副作用发生率差异无统计学意义(P>0.05).结论福多司坦与消坦立对COPD合并PF患者均有疗效,福多司坦在改善肺功能、抗肺纤维化、减轻氧化应激反应、提高生活质量方面更优. Objective To compare the application effects of Fudosteine and Acetylcysteine granules in the treatment of chronic obstructive pulmonary disease(COPD)with pulmonary fibrosis(PF).Method 96 patients with COPD and PF were selected and divided into observation group and control group according to the random number table method,with 48 cases in each group.On the basis of conventional treatment,observation group was given oral Fudosteine,and control group was given oral Acetylcysteine granules,and they were continuously treated for 6 months.The pulmonary function(forced expiratory volume in 1 s(FEV1),forced vital capacity(FVC),maximum peak expiratory flow rate(PEF)),pulmonary fibrosis degree(clinical,radiographic,physiological(CRP)score)and related serum indicators(laminin(LN),hyaluronic acid(HA),procollagen typeⅢ(PC-Ⅲ)),oxidative stress indicators(malondialdehyde(MDA),superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)),quality of life(St.George’s Respiratory Questionnaire(SGRQ))and medication side effects were compared between the two groups.Results After treatment,the pulmonary function indexes FEV1,FVC and PEF in the two groups were higher than those before treatment(P<0.05).The scores of dimensions of clinical symptoms,imaging performance and physiological status and total score of CRP scoring system were lower than those before treatment(P<0.05).The levels of serum LN,HA and PC-Ⅲwere lower than those before treatment(P<0.05).The level of serum MDA was lower than that before treatment(P<0.05)while the levels of SOD and GSH-Px were higher than those before treatment(P<0.05).The scores of respiratory symptom,disease influence and activity ability of SGRQ scale were lower than those before treatment(P<0.05),and the increases or decreases of the above indicators in observation group were greater than those in control group(P<0.05).There were no statistically significant differences in the incidence rates of medication side effects between the two groups(P>0.05).Conclusion Fudosteine and Acetylcysteine gra
