摘要
目的探讨全麻复合硬膜外阻滞麻醉对宫颈癌患者外周血T细胞亚群及术后苏醒质量的影响。方法将我院收治的140例宫颈癌患者根据简单随机方式分为对照组和观察组,各70例。两组均行腹腔镜手术,对照组予以全身麻醉(简称全麻),观察组予以全麻复合硬膜外阻滞麻醉。比较两组的麻醉优良率、七氟烷用量、苏醒质量、外周血T细胞亚群变化情况。结果观察组的麻醉优良率高于对照组(P<0.05)。观察组的七氟烷用量少于对照组,自主呼吸恢复时间、清醒时间均短于对照组(P<0.05)。术后24 h,两组的外周血CD3^+、CD4^+、CD4^+/CD8^+均较麻醉前下降,但观察组高于对照组(P<0.05);术后72 h,两组的外周血CD3^+、CD4^+、CD4^+/CD8^+均较术后24 h升高,且观察组高于对照组(P<0.05)。结论全麻复合硬膜外阻滞麻醉应用于宫颈癌患者中的麻醉效果良好,能有效减少麻醉药物用量,调节免疫抑制反应,提高苏醒质量。
Objective To investigate the effects of general anesthesia combined with epidural block anesthesia on peripheral blood T cell subsets and the quality of postoperative recovery in patients with cervical cancer.Methods A total of 140 patients with cervical cancer admitted in our hospital were divided into control group and observation group according to simple random method,with 70 cases in each group.Both groups underwent laparoscopic surgery,the control group was given general anesthesia,and the observation group was given general anesthesia combined with epidural block anesthesia.The excellent and good rate of anesthesia,the dosage of sevoflurane,the quality of recovery,the change of peripheral blood T cell subsets were compared between the two groups.Results The excellent and good rate of anesthesia in the observation group was higher than that in the control group(P<0.05).The dosage of sevoflurane in the observation group was less than that in the control group,recovery time of spontaneous breathing and awake time were shorter than those in the control group(P<0.05).At 24 h after surgery,CD3^+,CD4^+and CD4^+/CD8^+of peripheral blood in both groups decreased compared with those before anesthesia,but those in the observation group were higher than the control group(P<0.05);at 72 h after surgery,CD3^+,CD4^+and CD4^+/CD8^+of peripheral blood in the two groups all increased compared with those at 24 h after surgery,and those in the observation group were higher than the control group(P<0.05).Conclusion General anesthesia combined with epidural block anesthesia applied in cervical cancer patients has a good anesthesia effect,which can effectively reduce the dosage of anesthetic drugs,regulate the immune suppression response,and improve the quality of awake.
