摘要
目的探讨深层肌肉刺激(deep muscle stimulator,DMS)联合作业疗法对脑梗死患者腕关节功能的康复效果。方法选取2018年1月至2019年12月东南大学附属中大医院收治的72例脑梗死后腕关节功能障碍患者为研究对象。采用随机数字表法将入选患者分为观察组和对照组,每组各36例。对照组患者接受作业疗法干预,观察组患者接受DMS联合作业疗法干预,均持续4周。比较两组患者干预前后患侧上肢改良Ashworth分级、Fugl-Meyer量表评分、腕关节关节活动度(range of motion,ROM)、改良Barthel指数(modified barthel index,MBI)评分、脑卒中专用生存质量量表(stroke-specific quality of life,SS-QOL)评分。结果干预4周后,两组患者改良Ashworth分级均显著低于本组干预前(均P<0.05),Fugl-Meyer量表上肢评分、腕关节ROM、MBI评分和SS-QOL评分均显著高于本组干预前(均P<0.05);且观察组患者改良Ashworth分级显著低于同期对照组(P<0.05),Fugl-Meyer量表上肢评分,腕关节的ROM、MBI评分和SS-QOL评分均显著高于同期对照组(均P<0.05)。结论DMS联合作业疗法能够显著促进脑梗死患者腕关节功能的恢复,提高患者上肢运动功能、生活自理能力和生活质量,康复效果优于单纯作业疗法。
Objective To explore the effect of deep muscle stimulation(DMS)combined with occupational therapy on the rehabilitation of wrist function in patients with cerebral infarction.Method Seventy-two patients with wrist joint dysfunction after cerebral infarction from January 2018 to December 2019 were enrolled for the study and randomly divided into observation group and control group,with 36 cases in each group.Patients in control group received occupational therapy intervention,while patients in observation group received DMS combined with routine occupational therapy intervention,both lasted for 4 weeks.The modified Ashworth grade,Fugl-Meyer assessment score of upper limb of affected side,range of motion(ROM)of wrist joint,modified Barthel index(MBI)score and stroke-specific quality of life(SS-QOL)score were compared between the two groups before and after intervention.Result Four weeks after intervention,the modified Ashworth grade of patients in the two groups were significantly lower than those before intervention(all P<0.05),and the Fugl-Meyer assessment scores of upper limb of affected side,ROM of wrist joints,MBI scores,and SS-QOL scores were significantly higher than those before intervention(all P<0.05),the modified Ashworth grade in observation group was significantly lower than that in control group(P<0.05),the Fugl-Meyer assessment score of upper limb of affected side,ROM of wrist joint,MBI score,and SS-QOL score were significantly higher than those in control group(all P<0.05).Conclusion DMS combined with occupational therapy can significantly promote the recovery of wrist function in patients with cerebral infarction,improve the motor function of upper limb,self-care ability and quality of life.Its rehabilitation effect is better than occupational therapy alone.
