摘要
目的观察阿维A酯联合地蒽芬软膏及光化学疗法用于顽固性银屑病患者的疗效。方法选取2017年2月至2019年3月就诊于本院的顽固性银屑病患者80例作为主要研究对象,采用随机数字表法将患者分为对照组(40例)、观察组(40例)。对照组采用阿维A酯治疗,观察组采用阿维A酯联合地蒽芬软膏及光化学疗法治疗。比较两组患者临床疗效、复发率、不良反应、皮损面积评分及免疫功能水平变化。结果观察组患者临床疗效(痊愈15例、显效10例、有效12例、无效3例)水平显著优于对照组(P<0.05);观察组复发率15.00%显著低于对照组(40.00%)(P<0.05);两组患者的不良反应比较差异无统计学意义(P>0.05);两组治疗后PASI评分及血管内皮生长因子(VEGF)均较治疗前显著下降(均P<0.05),观察组治疗后PASI评分及VEGF水平较对照组下降更为显著(均P<0.05)。结论阿维A酯联合地蒽芬软膏及光化学疗法用于顽固性银屑病患者,能够明显改善临床疗效,减少皮肤损伤面积及VEGF水平,且并未显著增加不良反应,并降低患者复发风险。
Objective To observe the curative effect of etretinate combined with dithranol ointment and photochemotherapy in patients with refractory psoriasis.Methods A total of 80 patients with refractory psoriasis treated in our hospital from February 2017 to March 2019 were selected as the main research subjects,and the patients were divided into control group(40 cases)and observation group(40 cases)by the random number table method.The control group was treated with etretinate,and the observation group was treated with etretinate combined with dithranol ointment and photochemotherapy.The clinical efficacy,recurrence rate,adverse reactions,skin lesion area score,and immune function level changes were compared between the two groups.Results The clinical efficacy(15 cured cases,10 obviously effective cases,12 effective cases,3 ineffective cases)in the observation group was significantly better than that in the control group(P<0.05).The recurrence rate of the observation group was 15.00%(6/40),lower than that of the control group[40.00%(16/40)](P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).After treatment,PASI score and vascular endothelial growth factor(VEGF)level in the two groups were significantly lower than those before treatment(all P<0.05),the PASI score and VEGF level in the observation group were significantly lower than those in the control group(both P<0.05).Conclusion Etretinate combined with dithranol ointment and photochemotherapy in patients with refractory psoriasis can significantly improve the clinical efficacy,reduce the area of skin injury and VEGF level,and does not significantly increase adverse reactions,and reduces the risk of recurrence.
