摘要
目的通过研究DRGs在医院绩效考核中应用情况,为提升医院能力建设和服务水平提供决策支持。方法将医院病案系统中2017年1月1日—2019年12月31日的住院病案首页信息上传至北京市DRGs绩效管理平台,运用北京市DRGs绩效管理工具对医院的服务能力、效率和质量安全等进行对比分析。结果医疗服务能力三年同期比较,分析病例数为19011例、20590例、21285例;总权重数为16440、18416、20070,年平均增幅20.32%;DRGs组数为591、604、613,年平均增幅1.84%;病例组合指数(CMI)为0.86、0.89、0.94,年平均增幅4.55%;医疗服务效率三年同期比较,时间消耗指数为0.96、0.96、0.97,费用消耗指数为0.97、0.95、0.97,数值均小于1;质量与安全指标三年同期比较,低风险死亡率0.02%、0.00%、0.02%。结论DRGs应用在医院绩效考核后,医疗服务能力和效率提升,质量与安全改善。但诊疗技术水平还有较大差距,需要不断加强学科建设。
Objective To explore and practice the application of DRGs in hospital performance appraisal to provide decision support for improving hospital capacity building and service level.Methods The medical record home page information from January 1,2017 to December 31,2019 in the hospital medical record system is uploaded on Beijing DRGs performance management platform,and the Beijing DRGs performance management tool is employed to compare and analyze the service ability,efficiency and quality safety of the hospital.Results Compared with the same period of 2017,2018 and 2019,the number of cases analyzed is 19011,20590 and 21285,the total weight of medical service capacity is 16440,18416 and 20070,the annual average growth rate is 20.32%,the number of DRGs is 591,604 and 613,the annual average growth rate is 1.84%,and the case mix index(CMI)is 0.86,0.89 and 0.94,the annual average growth rate is 4.55%;compared with the same period of 2017,2018 and 2019,the service efficiency is 0.96,0.96 and 0.97,and the cost consumption index is 0.97,0.95 and 0.97,all values are less than 1;compared with the same period of 2017,2018 and 2019,the low-risk mortality rate is 0.02%,0%and 0.02%.Conclusion After DRGs applied in hospital performance appraisal,the indicators of medical service capacity and service efficiency have been increased,and the indicators of quality and safety have been improved.Therefore,there is need for the hospital to continuously strengthen its discipline construction and improve the overall level of diagnosis and treatment.
