摘要
目的探讨一期后路半脊椎切除联合短节段内固定治疗半椎体脊柱侧后凸畸形的临床疗效。方法纳入2012年1月~2016年1月于本院收治的30例半椎体脊柱侧后凸畸形患者,均采用一期后路半脊椎切除联合短节段内固定治疗,随访3年以上,记录围手术期手术指标,临床疗效和随访期并发症情况。结果所有患者均成功完成手术,手术用时(136.76±25.34)min、出血量(384.88±64.43)mL、住院时间(21.23±4.09)d、随访(4.22±0.94)年;术后7 d、术后1年、末次随访时,节段侧凸角、节段后凸角、上端固定椎倾斜角、代偿弯、冠状面躯干偏移均小于术前,但各项影像学指标较术后7 d均有所增加,差异有统计学意义(P<0.05);术后1例发生切口感染、1例出现螺钉切割椎弓根现象,无内固定失败、假关节形成、交界性后凸等严重并发症。结论一期后路半脊椎切除联合短节段内固定治疗半椎体脊柱侧后凸畸形融合节段少,脊柱矫形效果优良,内固定稳固,术后并发症少,是半椎体脊柱侧后凸畸形安全、有效的矫正治疗方案。
Objective To study the clinical efficacy of one-stage posterior semi-spine resection combined with short segmental internal fixation in the treatment of vertebral kyphosis.Methods Thirty patients with vertebral kyphosis treated in our hospital from January 2012 to January 2016 were selected in this study.All patients underwent one-stage posterior semi-spine resection combined with short-segment internal fixation for more than 3 years.Perioperative indicators,clinical efficacy and complications during follow-up were recorded.Results All patients successfully completed the operation and obtained complete follow-up.The operating time was(136.76±25.34)min,bleeding volume was(384.88±64.43)mL,hospitalization time was(21.23±4.09)d,follow-up was(4.22±0.94)years.At postoperative 7 days,1 year after surgery,the last follow-up,the segmental lobes,segmental kyphosis,upper fixed vertebral tilt angle,compensatory curve,coronal torso offset were less than preoperation,but the imaging indicators were increased compared with 7 days after surgery,the differences were statistically significant(P<0.05).One case had incision infection,1 case had screw-cut pedicle after operation.No internal fixation failure,pseudoarticular formation,borderline kyphosis and other serious complications were observed.Conclusion One-stage posterior semi-spine resection combined with short-segment internal fixation in the treatment of vertebral kyphosis has advantages of less vertebral deformity,excellent spinal orthopedic effect,stable internal fixation,less postoperative complications,which is a safe and effective corrective treatment plan for semi-vertebral kyphosis.
