摘要
目的探讨取消药品加成后影响脑梗死住院患者费用变化的主要支配因子,为合理控制住院费用提供参考依据。方法以某三甲医院2016—2018年2 849例脑梗死住院患者为研究对象,采用因子分析法,首先运用KM0和球形Barlett’s检验的方法进行变量适应性检验;接着提取影响住院费用的公因子,提取公因子采用主成分分析法,因子旋转采用Kaiser标准化最大方差法;最终确定支配住院费用的公因子,进行分析讨论。结果经因子分析方法计算得出在累计方差贡献率为100%的9个公因子中,有2个公因子特征值大于1,能够解释原信息的71.248%,且2个公因子间相关系数<0.1,由方差最大化旋转后因子载荷矩阵确定影响脑梗死患者住院费用的公因子是服务诊断治疗因子及材料和其他因子。结论控制医疗费用的不合理增长,应着力降低药品费用、影像学诊断费和材料费。
Objective To explore the main dominating factors that influence the changes in the cost of inpatients with cerebral infarction after the abolition of drug additions, and to provide a reference for reasonable control of hospitalization costs. Methods A total of two thousand eight hundred and forty-nine inpatients with cerebral infarction in a tertiary hospital from 2016 to 2018 were selected as the research objects, and the factor analysis method was used. First, the KM0 and spherical Barlett’s test were used to test the adaptability of variables;then the common factors affecting hospitalization costs were extracted. Principal component analysis was used to extract common factors, and Kaiser standardized maximum variance method was used for factor rotation;the common factors governing hospitalization expenses were finally determined and discussed. Results The factor analysis method calculates that among the 9 common factors with a cumulative variance contribution rate of 100%, 2 common factors have feature values greater than 1, which can explain 71.248% of the original information, and the correlation coefficient between the 2 common factors is less than 0.1. The common factors that affect the hospitalization expenses of patients with cerebral infarction are determined by the factor loading matrix after the maximum variance is rotated and are the service diagnosis and treatment factors, materials and other factors. Conclusion To control the unreasonable increase in medical expenses, efforts should be made to reduce the cost of medicines, diagnostic imaging and materials.
