摘要
目的研究探讨新生儿肺透明膜病(PHMD)的影像学表现特征,为临床进一步明确疾病诊断提供重要依据。方法回顾性分析于本院影像科就诊且经临床医生体格检查配合影像学辅助诊断确诊的27例PHMD患儿的临床资料,分析患儿的影像学变化特征,探讨影像学对PHMD的临床诊断价值。结果本研究中的27例患儿中,出现不同影像学改变,有的患儿同时出现多个改变,3例肺野部影像显示密度增高影呈现网格状改变及小斑点状改变,以肺野内中带显著,外带肺野透亮度增高;23例肺野透光度不同程度减低,呈现“毛玻璃”状改变;5例出现“白肺”改变;19例出现空气支气管征,心缘及膈面模糊不清;仅1例显示肺纹理增强。本研究并发气胸5例,纵隔气肿2例,肺出血4例,动脉导管未闭3例,慢性肺损伤1例。结论X线影像学检查是PHMD的主要检查方法。胸片显示肺野透光度减低、“白肺”及空气支气管征时结合临床表现可作出较明确的诊断。
Objective To study the imaging features of neonatal hyaline membrane disease(PHMD),so as to provide an important basis for clinical diagnosis.Methods The clinical data of 27 children with PHMD diagnosed by physical examination of clinicians and auxiliary diagnosis of imaging were retrospectively analyzed.The imaging characteristics of children was analyzed,and the clinical value of imaging diagnosis for PHMD was discussed.Results The 27 children in this study all had different imaging changes,and some children had multiple changes at the same time.The images of the lung fields of 3 children showed grid-like changes and small spot-like changes in the lung field,with the lung field in the middle zone significantly increased,the outer lung field permeability increased.The light transmittance of the lung field in 23 cases decreased to varying degrees,showing"ground glass"changes,5 cases showed"white lung"changes,19 cases showed air bronchial signs,and the heart border and diaphragm surface were blurred.Only 1 case showed enhanced lung texture.In this study,5 cases were complicated with pneumothorax,2 cases with mediastinal emphysema,4 cases with pulmonary hemorrhage,3 cases with patent ductus arteriosus,and 1 case with chronic lung injury.Conclusion X-ray imaging examination is the main examination method for PHMD.Chest radiograph showing reduced lung light transmittance,"white lung"and air bronchial signs combined with clinical manifestations can make a clearer diagnosis of PHMD.
