摘要
目的探讨健康教育对老年胃癌根治术患者营养状况和生活质量的影响。方法采用随机数字表法将120例老年胃癌根治术患者分为观察组和对照组,每组60例,对照组患者仅给予一般术后指导,观察组患者在对照组的基础上给予健康教育。比较干预前后两组患者的营养学指标[血清白蛋白(ALB)、转铁蛋白(TRF)、前白蛋白(PA)、血红蛋白(Hb)水平],采用欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)评价干预前后两组患者的生活质量。结果干预前,观察组和对照组患者的血清ALB、TRF、PA、Hb水平比较,差异均无统计学意义(P﹥0.05)。干预后,观察组患者的血清ALB、TRF、PA、Hb水平均明显高于对照组,差异均有统计学意义(P﹤0.01)。干预前,观察组和对照组患者EORTC QLQ-C30各维度评分比较,差异均无统计学意义(P﹥0.05)。干预后,观察组患者的躯体功能、角色功能、情绪功能、认知功能评分均高于对照组,疲乏、睡眠障碍、食欲丧失评分均低于对照组,差异均有统计学意义(P﹤0.05)。结论对老年胃癌根治术患者实施健康教育有利于改善患者术后生活质量,促进患者营养学指标的恢复。
Objective To explore the effect of health education on nutritional status and quality of life in elderly gastric cancer patients undergoing radical gastrectomy.Method 120 elderly gastric cancer patients undergoing radical gastrectomy were randomly divided into the control group(n=60,treated with general postoperative guidance)and the observation group(n=60,treated with health education on the basis of control group).Before and after intervention,the nutritional indicators[serum albumin(ALB),transferrin(TRF),prealbumin(PA)and hemoglobin(Hb)levels]was compared and the quality of life was evaluated by using European Organization for Research and Treatment of Cancer quality of life questionnaire core 30(EORTC QLQ-C30).Result Before intervention,there was no significant difference in levels of serum ALB,TRF,PA and Hb between the two groups(P>0.05).After intervention,the levels of serum ALB,TRF,PA and Hb in the observation group were higher than those in the control group(P<0.01).Before intervention,there was no significant difference for all dimensional scores of EORTC QLQ-C30(P>0.05).After intervention,the scores of physical function,role function,emotional function,cognitive function were higher and the scores of fatigue,sleep disturbance and loss of appetite were lower in the observation group than those in the control group(P<0.05).Conclusion Health education in elderly gastric cancer patients undergoing radical gastrectomy can improve the postoperative quality of life and promote the recovery of nutritional indicators.
