摘要
目的分析外周血调节性T细胞(Treg细胞)与吲哚胺2,3-双加氧酶(IDO)检测对慢性乙型肝炎病毒(HBV)感染者罹患原发性肝癌(PLC)的预测价值。方法选取2017年1月至2019年12月辽宁省金秋医院收治的188例慢性HBV感染者作为研究对象,其中84例单纯慢性乙型肝炎(CHB)患者作为CHB组,104例合并PLC患者作为PLC组,选取同期60名健康体检者作为对照组,检测各组的外周血Treg细胞和IDO信使RNA(mRNA)水平,分析外周血Treg细胞及IDO mRNA水平与HBV感染严重程度的相关性,探讨外周血Treg细胞及IDO mRNA在CHB患者罹患PLC中的预测价值。结果 CHB组和PLC组的外周血Treg细胞及IDO mRNA水平均显著高于对照组[(3.71±0.49)%、(11.43±1.57)%比(2.76±0.15)%,(1.34±0.25)、(2.91±0.60)比(0.68±0.12)],且PLC组高于CHB组(均P <0.05);Spearman相关分析结果显示,外周血Treg细胞及IDO mRNA水平与HBV感染严重程度呈正相关(r=0.397、r=0.425,均P <0.001);外周血Treg细胞预测慢性HBV感染者罹患PLC的灵敏度为86.5%、特异度为72.2%,外周血IDO mRNA预测慢性HBV感染者罹患PLC的灵敏度为87.4%、特异度为71.3%。结论外周血Treg细胞、IDO与HBV感染者发生PLC的风险呈正相关,两者对慢性HBV感染者罹患PLC具有较高的预测价值。
Objective To analyze the predictive value of regulatory T cells(Treg cells)and indoleamine 2,3-dioxygenase(IDO)detection for primary liver cancer(PLC)in patients with chronic hepatitis B virus(HBV)infection.Methods A total of 188 patients with chronic HBV infection in Liaoning Jinqiu Hospital from Jan.2017 to Dec.2019 were included,in which 84 patients with simple chronic hepatitis B(CHB)were divided into a CHB group,104 patients with PLC were divided into a PLC group,and 60 healthy persons who received physical examination during the same period were included as a control group.The levels of Treg cells and IDO messenger RNA(mRNA)in peripheral blood of each group were detected,and the correlation between the levels of Treg cells and IDO mRNA and the severity of HBV infection was analyzed,and the predictive value of Treg cells and IDO mRNA for PLC in patients with CHB was discussed.Results The levels of Treg cells in the CHB and PLC group were significantly higher than those in the control group[(3.71±0.49)%,(11.43±1.57)%vs(2.76±0.15)%;(1.34±0.25),(2.91±0.60)vs(0.68±0.12)],and those in the PLC group were higher than those in the CHB group(all P<0.05).The results of Spearman correlation coefficient analysis showed that the levels of Treg cells and IDO mRNA in peripheral blood were significantly positively correlated with the severity of HBV infection(r=0.397,r=0.425,all P<0.001).The sensitivity and specificity of Treg cells in predicting PLC in patients with chronic HBV infection were 86.5%and 72.2%,respectively;the sensitivity and specificity of IDO mRNA in peripheral blood for predicting PLC in patients with chronic HBV infection were 87.4%and 71.3%,respectively.Conclusion Peripheral blood Treg cells and IDO are positively correlated with the PLC risk in people with HBV infection,and both of them have high predictive value for PLC in chronic HBV infected people.
