针刀干预对KOA兔关节软骨细胞功能及细胞外基质代谢的影响 被引量:9

Effect of Acupotomy on Function of Articular Chondrocyte and Metabolism of Extracellular Matrix in Rabbit Model with Knee Osteoarthritis

摘要 目的:探索针刀干预对膝骨关节炎(knee osteoarthritis,KOA)模型兔关节软骨细胞功能及细胞外基质代谢的影响。方法:24只新西兰兔按体质量随机分为空白组(6只)及KOA造模组(18只),造模组应用改良的Videman法制动6周建立KOA兔模型,随后再根据Lequesne评分随机分为模型组、针刀组和西药组(每组6只)。各组分别进行干预治疗4周;治疗结束后,应用ELISA法检测血清、关节液中II型胶原C端肽(C-telopeptides of type II collagen,CTX-2)和软骨寡聚基质蛋白(cartilage oligomeric matrix protein,COMP)的含量,应用免疫荧光双标染色检测关节软骨细胞核、F-actin及Ⅱ型胶原的表达情况。结果:模型组血清CTX-2(P<0.01)、COMP(P<0.05)水平与空白组相比显著升高;干预后,针刀组和西药组血清CTX-2水平明显降低(P<0.05),针刀组和西药组血清COMP含量降低,但差异无统计学意义(P>0.05)。模型组关节液CTX-2、COMP水平与空白组相比表达明显升高(P<0.05);干预后,针刀组关节液CTX-2含量降低但差异无统计学意义(P>0.05),西药组关节液CTX-2含量表达降低(P<0.05),针刀组(P<0.01)和西药组(P<0.01)关节液COMP含量显著降低。模型组细胞核平均面积(P<0.01)、累积光密度(integral optical density,IOD)(P<0.01)与空白组相比显著升高,干预后,针刀组(P=0.852;P=0.318)和西药组(P=0.327;P=0.418)细胞核平均面积和细胞核IOD稍降低,但差异无统计学意义(P>0.05);模型组F-actin骨架蛋白IOD、核/F-actin IOD比值与空白组相比显著降低(P<0.01),干预后,针刀组和西药组骨架蛋白IOD、核/F-actin IOD比值与模型组相比表达升高(P<0.05;P<0.01);模型组Ⅱ型胶原IOD与空白组相比显著降低(P<0.01),干预后,针刀组和西药组Ⅱ型胶原IOD与模型组相比显著升高(P<0.01)。结论:针刀干预KOA兔可改善软骨细胞状态并缓解细胞外基质降解,从而发挥治疗作用。 Objective:To explore the effect of needle knife intervention on the function of articular cartilage cells and extracellular matrix metabolism in rabbits with knee osteoarthritis(knee osteoarthritis,KOA).Methods:Twenty-four New Zealand rabbits were randomly divided into blank group(6)and KOA model(18)according to their body weight.The model was immobilized with modified Videman method for 6 weeks to establish a KOA rabbit model,and then randomized according to Lequesne score Divided into model group,needle knife group and western medicine group(6 in each group).Each group received intervention treatment for 4 weeks;after the treatment,the serum,C-telopeptides of typeⅡcollagen(CTX-2)and cartilage oligomeric matrix protein(COMP)were detected by ELISA method.The content of COMP was used to detect the expression of articular chondrocyte nucleus,F-actin and typeⅡcollagen by immunofluorescence double-label staining.Results:The serum levels of CTX-2(P<0.01)and COMP(P<0.05)in the model group were significantly higher than those in the blank group;after intervention,the serum levels of the needle knife group(P<0.05)and the western medicine group(P<0.05)The expression of CTX-2 was significantly reduced,and the serum COMP content of the needle knife group(P=0.163)and the western medicine group(P=0.067)decreased,but the difference was not statistically significant.The expression of CTX-2 and COMP in the synovial fluid of the model group was significantly higher than that of the blank group(P<0.05);after intervention,the CTX-2 content in the synovial fluid of the needle knife group decreased but the difference was not statistically significant(P=0.156).The expression of CTX-2 in the joint fluid of the western medicine group decreased(P<0.05),and the content of COMP in the joint fluid of the acupotomy group(P<0.01)and the western medicine group(P<0.01)decreased significantly.The average nuclear area(P<0.01)and the integrated optical density(IOD)(P<0.01)of the model group were significantly higher than those of the blank gro
