摘要
目的高血压的发病率日趋增高,而部分患者合并焦虑状态时,增加了血压控制的难度。血流动力学检测为高血压患者的个性化治疗提供了行之有效的方法,但合并焦虑状态的高血压患者,尚无特异的血流动力学指标。本研究旨在分析左室内压最大变化速率(dp/dtlvmax)变化与高血压并焦虑状态的关系,为高血压的防治提供试验依据。方法选择2016年11月至2018年5月河南中医药大学第一附属医院心功能科行高血压分型(动态血压监测+血流动力学检查)检查的患者622例,根据dp/dtlvmax变化情况,分为试验组和对照组。分别统计24 h、白天、夜间平均收缩压和舒张压与dp/dtlvmax伴焦虑状态的关系。结果试验组与对照组24 h(χ2=12.802,RR=3.728,P=0.000<0.01)、白天(χ2=11.913,RR=3.548,P=0.001<0.001)、夜间(χ2=9.141,RR=3.100,P=0.002<0.001)平均收缩压均具有显著统计学意义;24小时(χ2=2.742,RR=1.66,P=0.041<0.05)、白天(χ2=4.288,RR=2.088,P=0.034<0.05)、夜间(χ2=4.182,RR=2.060,P=0.031<0.05)平均舒张压具有统计学意义。结论高血压合并焦虑状态时对血流动力学中dp/dtlvmax产生了相应的影响。
Objective The incidence of hypertension is increasing day by day, and some patients with anxiety state increase the difficulty of blood pressure control. Hemodynamics provides an effective method for the personalized treatment of hypertensive patients, but there is no specific hemodynamic index in hypertensive patients with anxiety.This study was designed to analyze the relationship between the change rate of left internal pressure(dp/dtlvmax) and hypertension and anxiety status, and provide experimental evidence for prevention and treatment of hypertension. Methods A total of 622 patients with hypertension syndrome(dynamic blood pressure monitoring + hemodynamics) were selected from the Department of Cardiology, First Affiliated Hospital of Henan University of Traditional Chinese Medicine from November 2016 to May 2018, according to dp/dtlvmax. The situation was divided into a test group and a control group. The relationship between 24-hour, daytime, nighttime mean systolic and diastolic blood pressure and dp/dtlvmax with anxiety status were calculated. Results The experimental group and the control group were 24 hours(χ~2=12.802, RR=3.728, P=0.000<0.01), daytime(χ~2=11.913, RR=3.548, P=0.001<0.001), nighttime(χ~2=9.141, RR=3.100, P=0.002<0.001) The mean systolic blood pressure was statistically significant;24 hours(χ~2=2.742, RR=1.66, P=0.041<0.05), daytime(χ~2=4.288, RR=2.088, P=0.034< 0.05), nighttime(χ~2=4.182, RR=2.060, P=0.031<0.05) The mean diastolic blood pressure was statistically significant. Conclusion Hypertension combined with anxiety has a corresponding effect on dp/dtlvmax in hemodynamics.
