摘要
[目的]调查下肢深静脉血栓病人出院后1年内自我管理情况及预后,为实施延续性护理提供依据。[方法]采用自行设计的下肢深静脉血栓病人自我管理调查问卷对169例深静脉血栓病人出院后第1个月、第3个月、第6个月和第12个月的自我管理及其预后情况进行调查,分析不同特征病人的自我管理情况。[结果]169例下肢深静脉血栓病人出院后共发生深静脉血栓后综合征(PTS)15例、静脉血栓栓塞(VTE)复发3例,病人知识管理、情绪管理、行为管理、环境管理及自我管理总均分随着出院时间延长逐渐降低,不同年龄、不同预后的病人出院后1年自我管理得分比较差异有统计学意义(P<0.05)。[结论]随着出院时间的延长,下肢深静脉血栓病人自我管理水平逐渐下降,且年龄及预后与病人自我管理水平有一定关系。医护人员应加强延续护理,提高病人出院后的自我管理水平,改善病人预后。
Objective:To investigate the self-management status and prognosis of patients with deep venous thrombosis in lower extremity within one year after discharge dynamically,so as to provide basis for continuous nursing.Methods:A total of 169 patients with deep venous thrombosis(DVT)were investigated on their self-management and prognosis at the 1 st,3 rd,6 th,and 12 th month after discharge by self-designed questionnaires.And the status of self-management in different characteristics of patients were also analyzed Results:15 cases of PTS,three cases of VTE recurrence occurred in 169 patients with lower extremity deep venous thrombosis after discharge.The knowledge management,emotional management,behavior management,environment management and personal management gradually decreased in a year.There were differences in self-management scores one year later among patients of different ages and prognosis(P<0.05).Conclusions:With the extension of discharge time,the self-management scores of patients with deep venous thrombosis of lower extremity decreased gradually.Moreover,there was a certain relationship between age and prognosis and selfmanagement of patients.Medical staff should take effective targeted intervention measures,strengthen continuous nursing,improve the level of self-management behavior of patients after discharge,improve the prognosis of patients.
