摘要
骨盆、髋臼骨折常继发于高能量损伤,损伤后骨折类型复杂多样,其治疗一直是骨科医师的棘手难题,也是近些年研究的热点。近年来,学者们对骨盆髋臼的研究从骨折分型、临床治疗及预后等方面不断进行探索,使骨科医师对骨盆、髋臼骨折有了全新的认识。尤其在诊治理念、损伤严重程度的判断、治疗方案的决策、内固定器材与微创技术的研发等方面已接近国际水平,有效降低严重骨盆、髋臼骨折的病死率、缩短治疗周期、改善治疗效果,提高了我国创伤骨科的整体水平。笔者对骨盆、髋臼骨折治疗理念、分型方法及手术治疗等几个方面的创新进行述评,旨在与骨科同仁分享当前主流治疗理念和创新性手术治疗策略。
Pelvic and acetabular fractures are often secondary to highenergy injuries.The fracture types after injuries are complex and varied.The treatment of acetabular fractures has always been a difficult problem for orthopedic surgeons and a hotspot of researches.In recent years,scholars have gradually made indepth researches on pelvic acetabulum fractures from aspects of fracture classification,clinical treatment and prognosis,making the orthopedic surgeons have a new understanding on pelvic and acetabular fractures.The concepts of diagnosis and treatment,judgment of injury severity,treatment decisionmaking,research and development of internal fixation apparatus and minimally invasive technologies have been near to the international level,effectively reducing the mortality rate of severe pelvic and acetabular fractures,shortening the treatment cycle,improving treatment effect and hence improving the overall level of orthopedic trauma in China.The author reviews innovations in the treatment concepts,classification methods and surgical treatments of pelvic and acetabular fractures,in order to share the current mainstream treatment concepts and innovative surgical treatment strategies with orthopedic colleagues.
