利培酮联合重复经颅磁刺激对首发精神分裂症的白质完整性分析及疗效研究

Integrity Analysis of White Matter in the Treatment of First Episode Schizophrenia with Risperidone Combined with Repetitive Transcranial Magnetic Stimulation

摘要 目的研究利培酮联合重复经颅磁刺激治疗首发精神分裂症的临床效果及对患者白质完整性的影响。方法选取2015年10月至2018年10月枣庄市精神卫生中心接受治疗的首发精神分裂症患者80例作为研究对象。采取随机数字表法将患者分为对照组和观察组,各40例。选用阳性和阴性症状量表(PANSS)评估治疗前、后患者评分结果差异,观察治疗期间患者不良反应发生率,评估患者白质各向异性分数变化值差异。结果治疗前,两组患者对比PANSS评分和白质各向异性分数比较,差异无统计学意义(P>0.05);治疗4周后,观察组PANSS量表阳性因子评分和白质各向异性分数对比均显著低于对照组(P<0.05)。两组患者治疗期间不良反应发生率比较,差异无统计学意义(χ^2=3.115,P=0.235)。结论利培酮联合重复经颅磁刺激治疗首发性精神分裂症的效果确切,可在短期内发挥功效,对改善患者精神性症状有积极作用。 Objective To study the clinical effect of risperidone combined with repetitive transcranial magnetic stimulation on white matter integrity in patients with first-episode schizophrenia.Methods A total of 80 first-episode schizophrenic patients treated in Zaozhuang Mental Health Center from October 2015 to October 2018 were selected as study subjects.The patients were divided into control group and observation group by random number table method,40 cases each.The positive and negative Symptom Inventory(PANSS)was used to evaluate the difference in patient scores before and after treatment,and the incidence of adverse events in patients during treatment was observed to evaluate the difference in the variation value of the white matter anisotropy score.Results Before treatment,there was no significant difference in PANSS score and white matter anisotropy score between the two groups(P>0.05);after 4 weeks of treatment,the positive factor score and white matter anisotropy score in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(χ^2=3.115,P=0.235).Conclusion Risperidone combined with repetitive transcranial magnetic stimulation is effective in the treatment of first-episode schizophrenia,and it can play an active role in improving the mental symptoms of patients in a short time.
