摘要
目的心电图和超声心动图的相关性分析以及联合诊断心衰的效果。方法选取江苏省东台市人民医院2018年1月至2019年1月收治的36例心衰患者作为研究组,并选取同期36例正常体检健康人员作为对照组。对两组患者的心电图以及超声心动图资料进行对比。结果与对照组人员相比较,研究组患者的左心球形指数、室壁运动评分以及左室质量增加评分均明显增加,而左室压力上升速率、心脏指数以及射血分数均有显著降低。绝大部分患者的心电图均存在心电图V1导联P波终末电势(PtfV1)异常现象,且阳性率明显高于其他心电图表现。并且绝大部分心衰患者在Q-T间期均存在明显的离散度异常现象以及左心室肥大现象。两种检查方式联合诊断对于心衰诊断具有显著相关性。联合诊断心力衰竭的敏感度以及特异度都达到97.22%,均明显高于单纯心电图或超声心动图检查(P<0.05)。结论心电图和超声心动图对心衰患者进行联合诊断具有明显相关性,联合诊断具有较高的敏感度以及特异度,对心衰患者的临床诊断中可以根据患者情况灵活运用心电图超声心动图,提高心衰诊治水平。
Objective To explore the effect of combined diagnosis of heart failure by correlation analysis of ECG and echocardiography.Methods Totally 36 patients with heart failure admitted to Dongtai People's Hospital from January 2018 to January 2019 were selected as the study group and 36 healthy People as the control group.ECG and echocardiography data of the two groups were compared.Results Compared with the control group,the left ventricular spherical index,wall motion score and left ventricular mass increase score in the study group were significantly increased,while the left ventricular pressure rise rate,heart index and ejection fraction were significantly reduced.Most of the patients had abnormal PtfV1,and the positive rate was significantly higher than other ECG manifestations.In addition,most of the patients with heart failure have obvious dispersion abnormality and left ventricular hypertrophy in Q-T interval.There was significant correlation between the two methods in the diagnosis of heart failure.The sensitivity and specificity of combined diagnosis of heart failure were 97.22%,which were significantly higher than those of ECG or echocardiography(P<0.05).Conclusion ECG and echocardiography have obvious correlation in the combined diagnosis of heart failure patients.The combined diagnosis has high sensitivity and specificity.In the clinical diagnosis of heart failure patients,ECG and echocardiography can be flexibly used according to the patients'situation to improve the level of diagnosis and treatment of heart failure.
