摘要
目的:探讨超声引导对重症患者足背动脉穿刺置管成功率的影响和对重症医学住院医师规范化培训学员的临床带教作用。方法:选取重症医学科收治的患者70例,采用随机数字表法分为两组,试验组和对照组,每组35例。70名学员分为两组(A组和B组),每组各35名。对照组由A组学员采用常规穿刺法进行足背动脉穿刺,试验组由B组学员采用超声引导下足背动脉穿刺。比较试验组与对照组一次穿刺成功率、一次穿刺成功时间、总体穿刺成功率、总体穿刺时间、穿刺并发症及两组学员的教学满意度。结果:与对照组比较,试验组一次穿刺成功率和总体穿刺成功率更高,一次穿刺时间及总体穿刺时间较长(P<0.05或P<0.01);穿刺血肿并发症显著降低(P<0.01);B组学员教学满意度明显高于A组(P<0.01)。结论:超声引导下足背动脉穿刺具有较高的穿刺成功率及较低的血肿并发症发生率,同时明显提高重症住院医师规范化培训学员的带教满意度,值得临床教学推广。
Objective:To investigate the effect of ultrasound-guiding on dorsal artery of foot puncture and clinical teaching for standardized residents training students in critical care medicine.Method:70 patients in ICU were selected and divided into two groups by using the random number method.The test group and the control group,included 35 patients in each group.70 students were divided into two groups,group A and group B,35 participants in each group.In the control group,students in group A were subjected to conventional dorsal artery of foot puncture;in the test group,students in group B were subjected to ultrasound-guided dorsal artery of foot puncture.The success rate of single puncture,the time of single puncture,the total success rate of puncture,the total time of puncture,complications and teaching satisfaction were recorded.Results:Compared with the control group,the success rate of single puncture and the total success rate in the test group were higher,the time of single puncture and the total time were longer(P<0.05 or P<0.01);the incidence of complication of hematoma was significantly reduced(P<0.01);compared with the control group,the teaching satisfaction of the test group increased(P<0.01).Conclusion:Ultrasound guided dorsal artery of foot puncture has a higher puncture success rate,a lower complications incidence of hematoma,and significantly improves the teaching satisfaction of standardized residents training students in critical care medicine,which is worthy of clinical teaching promotion.
