摘要
目的探讨血清胃蛋白酶原、胃泌素-17、癌胚抗原(CEA)、糖类抗原72-4(CA72-4)联合检测对胃癌的诊断价值。方法选取80例胃癌患者作为胃癌组,选取100例同期胃良性病变患者作为对照组。比较两组患者的血清胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ、胃泌素-17、CEA、CA72-4水平,以及单一血清学指标与联合检测诊断胃癌的灵敏度、特异度、准确度。结果两组患者的胃蛋白酶原Ⅱ水平比较,差异无统计学意义(P﹥0.05)。胃癌组患者的胃蛋白酶原Ⅰ水平明显低于对照组患者,胃泌素-17、CEA、CA72-4水平均明显高于对照组患者(P﹤0.01)。ROC曲线分析发现,单一血清学指标检测与联合检测诊断胃癌的特异度比较,差异无统计学意义(P﹥0.05)。血清胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ、胃泌素-17、CEA、CA72-4联合检测的ROC曲线下面积最大,对胃癌的诊断效能最高,其中,灵敏度为93.75%(75/80),特异度为94.00%(94/100),准确度为93.89%(169/180)。结论血清胃蛋白酶原、胃泌素-17、CEA、CA72-4联合检测诊断胃癌具有较高的灵敏度、特异度和准确度,对胃癌的筛查和诊断均具有重要意义。
Objective To analyze the clinical value of combined detection of serum pepsinogen, gastrin-17, carcinoembryonic antigen(CEA), carbohydrate antigen 72-4(CA72-4) in the diagnosis of gastric cancer. Method Eighty patients with gastric cancer were selected as the gastric cancer group, and 100 patients with benign gastric lesions at the same period were selected as the control group. The serum levels of pepsinogen Ⅰ, pepsinogen Ⅱ, gastrin-17, CEA and CA72-4 in the two groups were compared. The sensitivity, specificity and accuracy of single serological index and combined detection in the diagnosis of gastric cancer were compared. Result There was no significant difference in the levels of pepsinogen Ⅱ between the two groups(P>0.05). The level of pepsinogen Ⅰ in the gastric cancer group was significantly lower than that in the control group, and the levels of gastrin-17, CEA, CA72-4 were significantly higher than that in the control group(P<0.01). ROC curve analysis showed that there was no significant difference in the specificity of single serological index detection and combined detection in the diagnosis of gastric cancer(P>0.05). Serum pepsinogen Ⅰ,pepsinogen Ⅱ, gastrin-17, CEA, CA72-4 joint detection area under the ROC curve is the largest, highest efficiency to the diagnosis of gastric cancer, among them, the sensitivity was 93.75%(75/80), the specificity was 94.00%(94/100), and the accuracy was 93.89%(169/180). Conclusion The combined detection of serum pepsinogen, gastrin-17, CEA, CA72-4 can effectively improve the sensitivity and accuracy of gastric cancer diagnosis, and is of great significance for gastric cancer screening and early diagnosis.
