摘要
目的比较经皮椎体成形术(PVP)与经皮椎体后凸成形术(PKP)在骨质疏松脊柱压缩性骨折患者中的临床效果。方法选取2016年2月至2019年4月于本院接受治疗的52例骨质疏松性脊柱压缩性骨折患者作为研究对象,以治疗方式的不同分为PVP组(n=26)和PKP组(n=26),PVP组应用PVP治疗,PKP组应用PKP治疗。比较两组治疗情况(手术时间与骨水泥注射量),治疗前后脊椎功能、椎体高度与疼痛程度以及骨水泥渗漏率。结果在手术时间与骨水泥注射量比较上,PVP组分别短于、少于PKP组,差异有统计学意义(P<0.05)。治疗前,两组Oswestry评分、VAS评分比较差异无统计学意义;治疗后,两组上述评分均降低,但组间比较差异无统计学意义。治疗前,两组椎体高度比较差异无统计学意义;治疗后,两组椎体高度均升高,且PKP组高于PVP组,差异有统计学意义(P<0.05)。治疗后,PVP组骨水泥渗漏率为42.31%,高于PKP组的7.69%,差异有统计学意义(P<0.05)。结论在骨质疏松脊柱压缩性骨折患者中应用PVP与PKP治疗,均可促进患者疼痛缓解与脊椎功能改善,但PKP治疗后椎体高度与骨水泥渗漏情况优于PVP,而PVP手术时间与骨水泥注射量少于PKP,故需要临床结合情况为患者合理选择治疗方式。
Objective To compare the clinical effects of percutaneous vertebroplasty(PVP)and percutaneous kyphoplasty(PKP)in patients with osteoporotic vertebral compression fractures.Methods 52 patients with osteoporotic spinal compression fractures treated in our hospital from February 2016 to April 2019 were selected as study subjects,and were divided into PVP group(n=26)and PKP group(n=26)according to different treatment methods.PVP group was treated with PVP,while PKP group was treated with PKP.The treatment conditions(operation time and injection amount of bone cement),spinal function,vertebral body height and pain degree before and after treatment,and bone cement leakage rate were compared between the two groups.Results In comparison of operation time and injection volume of bone cement,PVP group was shorter than PKP group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in Oswestry scores and VAS scores between the two groups.After treatment,the above two scores of the two groups decreased,but the difference was not statistically significant.Before treatment,there was no significant difference in vertebral body height between the two groups.After treatment,the vertebral body height of both groups was improved,and the PKP group was higher than the PVP group,with statistically significant difference(P<0.05).After treatment,the bone cement leakage rate of PVP group was 42.31%,higher than that of PKP group(7.69%),with statistically significant difference(P<0.05).Conclusion Application of PVP in patients with osteoporotic spinal compression fractures treated with PKP holds that all can promote the improvement of the patients to alleviate pain and spinal cord function,but the PKP after treatment vertebral body height and bone cement leakage was superior to the PVP,while the PVP operation time and bone cement injection amount is less than the PKP,so need to combined with clinical choose the treatment for the patients.
