摘要
目的探讨急性脑出血患者心电图、空腹血糖(FBG)及血清心肌酶变化的关系。方法方便选择2014年1月—2019年1月于该院住院急性脑出血患者70例,设为脑出血组,对照组为同期69例非脑出血患者。检测并比较两组心电图变化、空腹血糖和血清心肌酶水平,观察急性脑出血与心电图变化、空腹血糖及血清心肌酶水平的关系。结果脑出血组心电图异常率81.42%,对照组心电图异常率49.27%,组间差异有统计学意义(χ~2=15.890,P<0.05)。急性脑出血破入脑室系统组患者心电图心室复极异常率55.55%,未破入脑室系统组患者心电图心室复极异常率53.48%,差异无统计学意义(χ~2=0.029,P>0.05)。脑出血组空腹血糖、血清心肌酶中肌酸激酶(CK)分别为(6.65±1.89)mmol/L及(158.74±132.18)U/L,明显高于对照组(5.75±1.81)mmol/L及(89.84±59.97)U/L,差异有统计学意义(t=2.869,3.967,P<0.05)。结论急性脑出血与心电图异常变化存在显著相关,但急性脑出血是否破入脑室系统与心电图心室复极无明显相关,心电图异常变化可适时间接评估早期脑组织受损情况并及时指导治疗。急性脑出血患者空腹血糖和血清心肌酶升高,两者在评价急性脑出血病情轻重的判断中有重要意义。
Objective To explore the relationship between electrocardiogram(ECG),fasting blood glucose(FBG)and serum myocardial enzymes in patients with acute cerebral hemorrhage.Methods From January 2014 to January 2019,70 patients with acute cerebral hemorrhage were conveniently selected as the cerebral hemorrhage group,and the control group was 69 non-cerebral hemorrhage patients during the same period.The changes of ECG,fasting blood glucose and serum myocardial enzyme levels were detected and compared between the two groups,and the relationship between acute cerebral hemorrhage and changes of ECG,fasting blood glucose and serum myocardial enzyme levels were observed.Results The abnormal rate of ECG in the cerebral hemorrhage group was 81.42%,and the abnormal rate of ECG in the control group was 49.27%.The difference between the groups was statistically significant(χ2=15.890,P<0.05).The abnormal rate of ECG ventricular repolarization in patients with acute cerebral hemorrhage breaking into the ventricular system was 55.55%,and the abnormal rate of ventricular repolarization in patients without breaking into the ventricular system was 53.48%,the difference was not statistically significant(χ2=0.029,P>0.05).In the cerebral hemorrhage group,fasting blood glucose and serum myocardial enzyme creatine kinase(CK)were(6.65±1.89)mmol/L and(158.74±132.18)U/L,which were significantly higher than those in the control group(5.75±1.81)mmol/L and(89.84±59.97)U/L,the difference was statistically significant(t=2.869,3.967,P<0.05).Conclusion Acute cerebral hemorrhage is significantly related to abnormal changes in ECG,but whether acute cerebral hemorrhage breaks into the ventricular system is not significantly related to ECG ventricular repolarization.Abnormal changes in ECG can be used to assess early brain damage and promptly guide treatment.Elevated fasting blood glucose and serum myocardial enzymes in patients with acute cerebral hemorrhage are of great significance in evaluating the severity of acute cerebral hemorrhage.
