摘要
目的:分析延续性护理对2型糖尿病患者生活质量的影响,为其临床应用提供相关依据。方法:检索1979-01-01~2018-12-01国内外有关延续性护理对2型糖尿病患者生活质量影响的随机对照文献。根据改良后的Jadad量表进行文献质量评价,采用RevMan 5.3软件进行Meta分析。结果:共纳入19篇随机对照文献。Meta分析结果显示,延续性护理能显著降低2型糖尿病患者的空腹血糖[SMD=-1.79,95%CI(-2.26,-1.32),P<0.01]、餐后2h血糖[SMD=-1.41,95%CI(-1.93,-0.89),P<0.01]和糖化血红蛋白[SMD=-1.27,95%CI(-1.93,-0.60),P <0.01],并提高患者生活质量。生理职能[SMD=2.45,95%CI(1.63,3.27),P<0.01]、总体健康[SMD=2.09,95%CI(0.60,3.57),P<0.01]、精神健康[SMD=3.30,95%CI(2.10,4.50),P<0.01]、情感职能[SMD=2.42,95%CI(1.63,3.21),P<0.01]、社会功能[SMD=2.14,95%CI(1.49,2.78),P<0.01]、生理功能[SMD=2.40,95%CI(1.03,3.78),P<0.01]、活力[SMD=2.62,95%CI(1.78,3.46),P<0.01]、躯体疼痛[SMD=2.14,95%CI(0.74,3.54),P<0.01]得分均优于常规护理干预。结论:延续性护理有利于控制血糖稳定,改善2型糖尿病患者的生活质量。
Objective:To evaluate the influence of continuous care on the life quality in patients with type 2 diabetes mellitus,and to provide evidence for its clinical application.Methods:The randomized controlled trials(RCTs)on the effect of continuous care on the quality of life in patients with type 2 diabetes mellitus were searched from January 1979 to December 2018.The literature quality was evaluated according to the modified Jadad scale,and Meta-analysis was performed through RevMan 5.3 software.Results:Nineteen RCTs were included.Meta-analysis showed that continuous care significantly reduced fasting blood glucose in patients with type 2 diabetes mellitus,2 hours postprandial blood glucose and glycosylated hemoglobin,and improved the quality of life.Physiological functions,overall health,mental health,emotional function,social function,physiological function,viability,and somatic pain were better compared with conventional nursing intervention.Conclusion:Continuous care is conductive to stable blood glucose and improve the quality of life in patients with type 2 diabetes mellitus.
