不同灸疗方案对蛋白质能量消耗血透患者氨基酸与肉碱代谢的影响 被引量:5

Effects of different moxibustion schemes on amino acid and carnitine metabolism in hemodialysis patients with PEW

摘要 目的:观察不同灸疗方案对血透蛋白质能量消耗(PEW)患者氨基酸与肉碱代谢的影响。方法:纳入45例患者,随机分为对照组、调理脾胃组及辨证施灸组,对照组予西医常规治疗,调理脾胃组、辨证施灸组在此基础上分别加"调理脾胃灸法""辨证施灸方案",治疗12周,治疗前后检测氨基酸和酰基肉碱的水平。结果:调理脾胃组、辨证施灸组治疗后丙氨酸(Ala)、亮氨酸(Leu)、缬氨酸(Val)、鸟氨酸(Orn)、苯丙氨酸(Phe)水平明显高于治疗前(P<0.05),在Ala的改善方面,辨证施灸组优于调理脾胃组(P<0.05);对照组、调理脾胃组癸酰肉碱治疗后明显降低(P<0.05),辨证施灸组无显著性差异;对照组3-羟基-十八碳烯酰肉碱治疗后明显下降,调理脾胃组无明显变化,辨证施灸组治疗后显著上升(P<0.05)。结论:灸疗可能通过改善部分氨基酸和肉碱的代谢来改善血透患者PEW,辨证施灸方案效应靶点更多。 Objective:To observe the effects of different moxibustion schemes on amino acid and carnitine metabolism in hemodialysis patients with protein-energy wasting(PEW).Methods:Forty-five hemodialysis patients with PEW were randomly divided into three groups:the harmonizing spleen-stomach group,the individualized moxibustion scheme group and the control group.All groups received conventional western treatment methods.Additionally,the harmonizing spleen-stomach group received moxibustion program to harmonize spleen-stomach,and the individualized moxibustion group received individualized moxibustion.Treatment lasted 12 weeks.Amino acids and carnitines before and after were observed and recorded.Results:The levels of Ala,Leu,Val,Orn and Phe were obviously increased compared with before(P<0.05)in harmonizing spleen-stomach group and individualized moxibustion group.And the level of Ala in individualized moxibustion group was obviously increased compared with harmonizing spleen-stomach group(P<0.05).The levels of C10 were lower than that before treatment in control group and harmonizing spleen-stomach group(P<0.05),and there was no significant difference in individualized moxibustion group.The level of C18:1 OH was obviously decreased compared with before(P<0.05)in control group,but there were no significant difference in harmonizing spleen-stomach group,whereas,it was obviously increased compared with before in the control group individualized moxibustion group(P<0.05).Conclusion:Moxibustion may improve the PEW state of dialysis patients by improving the metabolism of some amino acids and carnitine in blood.The scheme individualized moxibustion seems to have more effective targets.
