摘要 目的评价3.0T高分辨磁共振血管壁成像(HR-MRI)在评估急性脑梗死(ACI)颅内动脉情况中的应用价值。方法回顾2018年4月—2019年5月的65例ACI病例情况,所有病例均行HR-MRI、DSA检查。以DSA结果为准,评判HR-MRI对ACI患者颅内动脉狭窄、闭塞的检出情况。结果65例患者经DSA检查共发现715条血管,其中正常562条,狭窄或闭塞共153条,DSA结果为标准,HR-MRI的敏感度71.89%、特异度94.66%、误诊率5.34%、漏诊率28.10%。DSA检出ACA总计111条血管、MCA总计108条血管、PCA总计106条血管、TICA总计132条血管、VA总计128条血管、BA总计130条血管;从各位置的血管狭窄情况来看,仍然以DSA检查结果为金标准,HR-MRI在MCA和PCA、BA等部位的狭窄血管检出情况与DSA对比,P>0.05。结论采用HR-MRI检查ACI患者颅内动脉情况,能有效检出颅内动脉狭窄、闭塞情况,并帮助确定病变部位,为ACI的诊断、预后评估提供可靠依据。 Objective To observe the application value of 3.0T highresolution MR angiography in the assessment of intracranial artery in acute cerebral infarction.Methods 65 cases of ACI from April 2018 to May 2019 were reviewed.All cases were examined by HR-MRI and DSA.With DSA as the gold standard,HR-MRI was used to evaluate the detection of intracranial artery stenosis and occlusion in ACI patients.Results In 65 patients,715 vessels were found by DSA,including 562 normal vessels and 153 stenosis or occlusion vessels.The sensitivity,specificity,misdiagnosis rate and missed diagnosis rate of HR-MRI were 71.89%,94.66%,5.34%and 28.10%,respectively.DSA detected a total of 111 vessels in ACA,108 vessels in MCA,106 vessels in PCA,132 vessels in TICA,128 vessels in VA and 130 vessels in Ba;HR-MRI detected stenosis in MCA,PCA and BA compared with DSA,P>0.05.Conclusion HR-MRI can effectively detect the stenosis and occlusion of the intracranial arteries in patients with ACI,and help to determine the location of the lesions,providing a reliable basis for the diagnosis and prognosis evaluation of ACI.
