上臂式输液港与胸壁式输液港有效性及安全性的Meta分析 被引量:6

Meta-analysis of the effectiveness and safety of upper arm type infusion port and chest wall type infusion port

摘要 目的通过Meta分析探讨上臂式输液港与胸壁式输液港的有效性和安全性。方法检索Cochrane Library、Pubmed、EM-base、CBM数据库、万方数据库、维普数据库及中国知网数据库,查找有关上臂式输液港与胸壁式输液港的对照研究,通过Revman 5.3软件对结局指标进行Meta分析与描述。检索时间为建库至2020年2月。结果共纳入16篇文献,包含3280例研究对象。Meta分析显示,在气胸[OR=0.24,95%CI(0.06,0.96),Z=2.01,P=0.04]、动脉损伤[OR=0.29,95%CI(0.11,0.76),Z=2.53,P=0.01]、血栓[OR=0.55,95%CI(0.33,0.91),Z=2.3,P=0.02]、导管阻塞[OR=0.38,95%CI(0.16,0.88),Z=2.25,P=0.02]、导管夹闭综合征[OR=0.14,95%CI(0.03,0.75),Z=2.3,P=0.02]并发症发生率上,上臂式输液港低于胸壁式输液港。在置管时间、置管成功率、导管异位、改变穿刺入路、感染、底座渗液、导管断裂、血肿并发症上,两组差异无统计学意义(P>0.05)。上臂组舒适度及生活质量评分高于胸壁组(P<0.05)。结论上臂式输液港不会增加患者输液港并发症发生率,且舒适度更高。 Objective Through Meta analysis,the effectiveness and safety of upper arm infusion port and chest wall infusion port were discussed to provide evidence for the best approach to intravenous infusion port.Methods Cochrane Library,Pubmed,EM-base,CBM database,Wanfang database,VIP database and CNKI database were searched to find the comparative study of upper arm infusion port and chest wall infusion port,and the outcome indicators were Meta analyzed and described by Revman 5.3 software.Results A total of 16 literatures were included,including 3280 subjects.Results of Meta analysis showed that in pneumothorax[OR=0.24,95%CI(0.06,0.96),Z=2.01,P=0.04],arterial injury[OR=0.29,95%CI(0.11,0.76),Z=2.53,P=0.01],thrombus[OR=0.55,95%CI(0.33,0.91),Z=2.3,P=0.02],catheter obstruction[OR=0.38,95%CI(0.16,0.88),Z=2.25,P=0.02],catheter clamping syndrome[OR=0.14,95%CI(0.03,0.75),Z=2.3,P=0.02],the incidence of complications,the upper arm infusion port is lower than the chest wall infusion port.During catheterization time,success rate of catheterization,catheter ectopic,change of puncture approach,infection,base effusion,catheter rupture,hematoma,complications were not statistically significant(P>0.05).The score of comfort and quality of life of upper arm group was higher than that of chest wall group(P<0.05).Conclusion The upper arm infusion port does not increase the incidence of complications in the infusion port,and the comfort level is higher.
