摘要
目的MGAP评分法(Mechanism,Glasgow Coma Scale,Age,and Arterial Pressure)是创伤患者的新方法,但其准确性还缺乏证据支持。本研究旨在评估其在多发伤患者死亡风险评估中的价值。方法纳入2015年4月16日0:00-2016年4月16日24:00,就诊于四川大学华西医院急诊科,共计375例,分别通过计算入院时的MGAP评分、CRAMS评分(Circulation,Respiration,Abdomen,Motor,and Speech score)、创伤严重度评分(Injury Severe Score,ISS),快速急诊医学评分(Rapid Emergency Medicine Score,REMS)、Sacco评分,qSOFA(The quick Sepsis-related Organ Falure Asessment)评分,以最终死亡为标准对以上创伤评分方法制作受试者工作曲线(ROC曲线),计算曲线下面积,并与损伤严重度评分(ISS评分)进行相关性分析。结果对于多发伤患者28 d死亡ROC曲线分析得出MGAP评分、CRAMS评分、qSOFA评分、REMS评分、Sacco评分评估多发伤患者28 d病死率的AUC分别是0.807、0.800、0.785、0.780、0.773。结论MGAP评分评估方法较简单,可推荐用于多发伤患者的预后评估。
Objective T Objective MGAP scoring method(Mechanism,Glasgow Coma Scale,Age,and Arterial Pressure)is a new method for trauma patients,but its accuracy is still lack of evidence support.The purpose of this study was to evaluate its value in the risk assessment of death in patients with multiple injuries.Methods A total of 375 patients were enrolled in the emergency department of West China Hospital of Sichuan University from 06:00 on April 16,2015 to 24:00 on April 16,2016.The MGAP score and CRAMS score(Circulation,Respiration,Abdomen,Motor,and Speech score),Injury Severe Score(ISS),Rapid Emergency Medicine Score(REMS),Sacco score,qSOFA(The quick Sepsis-related Organ Failure Assessment)score,The final death was the standard.A receiver working curve(ROC curve)was prepared for the above trauma scoring method,the area under the curve was calculated,and correlation analysis was performed with the injury severity score(ISS score).Results The 28-day death ROC curve analysis of multiple trauma patients obtained MGAP score,CRAMS score,qSOFA score,REMS score,and Sacco score.The AUC for evaluating the 28-day mortality of multiple trauma patients was 0.807,0.800,0.785,0.780,0.773,respectively.Conclusion The MGAP score evaluation method is relatively simple and can be recommended for the prognosis evaluation of patients with multiple injuries.
