The value of AMY, NLR and GP-2α in the severity evaluation of acute pancreatitis and their influence on the prognosis

摘要 目的探讨淀粉酶(AMY)、中性粒细胞/淋巴细胞比值(NLR)、糖蛋白-2α(GP-2α)在急性胰腺炎病情评估中的价值及对预后的影响。方法选取92例AP患者作为研究对象,根据严重程度不同分为MAP组(n=44)、MSAP组(n=30)和SAP组(n=18);同期选取50例健康体检者作为对照组,对比各组AMY、NLR和GP-2α差异,应用Pearson相关分析AMY与NLR和GP-2α之间的关系;绘制受试者工作曲线(ROC)探讨AMY、NLR和GP-2α各单项指标及联合指标在AP病情评估中价值;应用COX回归分析探讨AMY、NLR和GP-2α对SAP预后的影响。结果对照组、MAP组、MSAP组和SAP组四组NLR、AMY和GP-2α比较,差异有统计学意义(P<0.05);其中SAP组高于MSAP组(P<0.05),MSAP组高于MAP组(P<0.05),MAP组高于对照组(P<0.05)。经Pearson相关分析,AMY与NLR和GP-2α均呈明显正相关(P<0.001)。绘制ROC曲线发现,NLR和AMY单项指标在鉴别MAP组、MSAP组和SAP组,差异无统计学意义(P>0.05),而GP-2α单项指标可鉴别MAP组、MSAP组和SAP组(P<0.05);联合检测均可鉴别MAP组、MSAP组和SAP组(P<0.05),以AMY+NLR+GP-2α效果最佳,其灵敏度和特异度分别为86.3%和82.5%。经COX回归分析,AMY、NLR和GP-2α升高,SAP死亡风险增加(P<0.05)。结论AMY、NLR、GP-2α在评估AP病情严重程度中具有一定价值,且可影响SAP患者预后。 Objective To explore the value of amylase(AMY),neutrophil/lymphocyte ratio(NLR)and glycoprotein-2α(GP-2α)in the severity evaluation of acute pancreatitis(AP)and their influence on the prognosis.Methods A total of 92 patients with AP were selected as the subjects of research and divided into the MAP group(n=44),the MSAP group(n=30)and the SAP group(n=18)according to their severity.At the same time,50 people having health checkup were selected as the control group.The differences in AMY,NLR and GP-2αwere compared among all the groups.Pearson correlation was used to analyze the relationship among AMY,NLR and GP-2α.The receiver operating curve(ROC)was drawn to explore the value of the single indexes of AMY,NLR and GP-2αand the combined indexes in the severity evaluation of AP.COX regression analysis was used to explore the influence of AMY,NLR and GP-2αon the prognosis of SAP.Results NLR,AMY and GP-2αwere compared among the control group,the MAP group,the MSAP group and the SAP group,and the differences were statistically significant(P<0.05);among the comparisons,those of the SAP group were higher than those of the MSAP group(P<0.05),those of the MSAP group were higher than those of the MAP group(P<0.05),and those of the MAP group were higher than those of the control group(P<0.05).Pearson correlation analysis showed that AMY,NLR and GP-2αwere all obviously positively correlated(P<0.001).Through drawing ROC curve,it was found that the single indexes of NLR and AMY showed no statistically significant differences in identifying the MAP group,the MSAP group and the SAP group(P>0.05),while the single index of GP-2αcould identify the MAP group,the MSAP group and the SAP group(P<0.05);all the combined detections could identify the MAP group,the MSAP group and the SAP group(P<0.05),and among them AMY+NLR+GP-2αwas the best,with a sensitivity of 86.3%and a specificity of 82.5%.COX regression analysis showed that AMY,NLR and GP-2αincreased,and the risk of SAP death became greater(P<0.05).Conclusion AMY,NLR and GP-2�
