急性脑梗死患者外周血PLR、NLR水平与神经功能修复的相关性 被引量:5

Correlation between PLR and NLR levels in peripheral blood and neurological restoration in patients with acute cerebral infarction

摘要 目的分析急性脑梗死患者外周血血小板与淋巴细胞比值(PLR)、中性粒细胞与淋巴细胞比值(NLR)与神经功能恢复的关系。方法回顾性分析2018年1月至2020年1月商丘市第三人民医院收治的80例急性脑梗死患者的临床资料,并依据治疗前后美国国立卫生研究院卒中量表(NIHSS)评分减分率判定患者神经功能恢复情况,并据此分为恢复良好组(63例)与恢复不良组(17例)。记录并比较两组血小板与淋巴细胞比值(PLR)、中性粒细胞与淋巴细胞比值(NLR),分析PLR、NLR与神经功能恢复的关系。结果相较于恢复不良组,恢复良好组患者PLR、NLR参数水平均较低,差异有统计学意义(P<0.05);相关性检验结果显示,急性脑梗死患者NIHSS评分与PLR、NLR均呈正相关(r>0,P<0.05);线性回归分析得到回归方程:YNIHSS=2.931-0.096XPLR+10.540XNLR,结果显示急性脑梗死患者血清PLR、NLR升高可能是急性脑梗死患者治疗后神经功能恢复不良的影响因素(P<0.05);绘制受试者工作特征曲线(ROC)结果显示,PLR、NLR预测急性脑梗死患者神经功能恢复不良的曲线下面积(AUC)分别为0.782、0.884,均>0.75,有一定理想的预测效能。结论急性脑梗死患者普遍存在外周血NLR、PLR高表达,这可能是影响患者神经功能恢复的因素之一,且NLR、PLR值越高,患者神经功能恢复情况越差,临床可考虑将监测急性脑梗死患者的外周血PLR、NLR水平来预测患者神经功能缺损情况,指导早期干预与防治。 Objective To analyze the relationship between peripheral blood platelet-to-lymphocyte ratio(PLR),neutrophil-to-lymphocyte ratio(NLR)and neurological function recovery in patients with acute cerebral infarction.Methods A retrospective analysis was conducted to collect clinical data of 80 patients with acute cerebral infarction who had completed the treatment between January 2018 and January 2020 in Shangqiu Third People's Hospital,and determined the recovery of neurological function based on the reduction rate of National Institutes of Health Stroke Scale(NIHSS)scores before and after treatment.According to this,they were divided into good recovery group(63 cases)and poor recovery group(17 cases).The levels of PLR and NLR between the two groups were recorded and compared,and the relationship between PLR and NLR and neural function recovery was analyzed.Results Compared with the poor recovery group,the PLR and NLR parameters of the well recovery group were lower,and the difference was statistically significant(P<0.05);the correlation test results showed that the NIHSS score and PLR and NLR in patients with acute cerebral infarction were positive correlation(r>0,P<0.05);Linear regression analysis obtained the regression equation:YNIHSS=2.931-0.096XPLR+10.540XNLR,The results showed that the elevated serum PLR and NLR in patients with acute cerebral infarction may be the influencing factors of poor recovery of neurological function after treatment in patients with acute cerebral infarction(P<0.05);the results of drawing the receiver operating curve(ROC)showed that the areas under the curve(AUC)obtained by PLR and NLR to predict poor neurological function recovery in patients with acute cerebral infarction were:0.782 and 0.884,all>0.75,there was a certain ideal prediction performance.Conclusions The high expression of NLR and PLR in peripheral blood is common in patients with acute cerebral infarction,which may be one of the factors that affect the recovery of neurological function.And the higher the value of NLR and P
