摘要
目的:探讨雷珠单抗玻璃体内注射对新生血管性青光眼手术患者眼压及功能性滤过泡的影响。方法:回顾性分析2018年1月—2019年8月收治的90例新生血管性青光眼患者的临床资料,所有患者均采用注射雷珠单抗+改良小梁切除术治疗。记录注射雷珠单抗前、注射3 d后、术后1周、术后1个月、术后2个月患者眼压、视力变化情况;比较术后1个月、术后2个月功能性滤泡情况;记录术后并发症。结果:注射3 d后、术后1周,患者眼压较注射前呈下降趋势,视力呈升高趋势;术后1个月、术后2个月,患者眼压较术后1周有回升,视力有降低,但眼压仍低于注射前,视力高于注射前。各时点眼压、视力行重复度量单因素方差分析检验,差异均有统计学意义(P<0.05);术后1个月、2个月,功能性滤泡分别为88.89%,96.67%,差异无统计学意义(P>0.05)。术后无严重并发症。结论:新生血管性青光眼手术患者采用雷珠单抗玻璃体内注射,可改善眼压、视力,术后滤过泡形成效果明显,并发症少。
Objective:To investigate the effect of intravitreal injection of ranibizumab on intraocular pressure and functional filtering bleb in patients with neovascular glaucoma surgery.Methods:The clinical data of 90 patients with neovascular glaucoma who admitted in the hospital from January 2018 to August 2019 were retrospectively analyzed.All patients underwent ranibizumab injection and modified trabeculectomy.The changes of intraocular pressure and vision before ranibizumab injection,after injection for 3d,after surgery for 1 week,after surgery for 1 month and after surgery for 2 months were recorded.The functional filtering bleb after surgery for 1 month and 2 months was compared.The postoperative complications were recorded.Results:After injection for 3d and after surgery for 1 week,the intraocular pressure showed decreased trend compared with before injection,and the vision showed increased trend;After surgery for 1 month and 2 months,the intraocular pressure of patients was higher than that after surgery for 1 week,the vision decreased,but the intraocular pressure was still lower than that before injection,the vision was still higher than that before injection,the intraocular pressure and vision at each time were given repeated measures one-way analysis of variance test,and the differences were statistically significant(P<0.05).The rates of functional filtering bleb after surgery for 1 month and 2 months were 88.89%and 96.67%,there was no significient difference(P>0.05).and there were no severe postoperative complications.Conclusion:Intravitreal injection of ranibizumab in patients with neovascular glaucoma surgery can improve intraocular pressure and vision,have significant effects of postoperative formation of filtering bleb and less complications.
