摘要
目的:探讨慢性心力衰竭患者血清同型半胱氨酸(Hcy)水平与心房颤动(以下简称房颤)的相关性。方法:选择慢性心力衰竭并房颤患者(A组)、慢性心力衰竭并窦性心律患者(B组)、房颤无慢性心力衰竭患者(C组)及无房颤无慢性心力衰竭患者(D组)各52例。比较各组间生化指标、心脏彩超结果及血清Hcy水平。结果:四组相关生化指标(血糖、血脂、心肌酶谱)以及心脏超声参数(左室内径、室间隔)比较,差异无统计学意义(P>0.05)。左房内径(LAD)及Hcy水平均示D组<B组<C组<A组,且除B组血清Hcy水平与C组比较差异无统计学意义(P>0.05)外,其余各组比较差异均有统计学意义(P<0.05)。以A组为例进行分析,A组中血清Hcy与LAD呈正相关,且差异有统计学意义(r=0.332,P<0.05)。血清Hcy的受试者工作特征(ROC)曲线下面积为0.724,95%的可信区间为0.615~0.833(P<0.01)。结论:血清Hcy水平可能与慢性心力衰竭患者发生心房颤动具有相关性。
Objective:Preliminary explored the relationship between the level of serum homocysteine(Hcy)and atrial fibrillation(hereinafter referred to as atrial fibrillation)in patient with chronic heart failure.Methods:Selected chronic heart failure and atrial fibrillation patients 52 cases,Chronic heart failure and sinus rhythm patients 52 cases,atrial fibrillation and non-heart failure patients 52 cases,non-heart failure and non-atrial fibrillation patients during the same period 52 cases as controls.Comparison the biochemical indicators,cardiac color ultrasound date and serum Hcy between groups.Results:The four group had no statistical difference on related biochemical indicators(blood glucose,blood lipids,myocardial enzyme spectrum),and echocardiographic parameters(Left ventricular diameter,ventricular septum)(P>0.05).Left atrium inner diameter(LAD)and Hcy level showed that group D<group B<group C<group A,and there was no significant difference in serum Hcy level between group B and group C(P>0.05),the other groups were compared The differences were statistically significant(P<0.05).Serum Hcy and LAD in group A were positively correlated,and the difference was statistically significant(r=0.332,P<0.05).Taking group A as the experimental group to analyze the receiver operating characteristic(ROC)curve of serum Hcy,the area under the curve was 0.724,and the 95%confidence interval was 0.615~0.833(P<0.01).Conclusion:Serum Hcy levels may be associated with atrial fibrillation in patients with chronic heart failure.
