摘要
目的探讨合并外伤性蛛网膜下腔出血的颅脑损伤患者采用颅内压监护下脑室外引流联合腰穿治疗的效果。方法选取2018年2月至2019年2月本院收治的60例合并外伤性蛛网膜下腔出血的颅脑损伤患者为研究对象,采用单双号抽签方式分组,抽取到单号的患者纳入对照组,抽取到双号的患者纳入观察组,各30例。对照组采用常规方式治疗,观察组采用颅内压监护下脑室外引流联合腰穿治疗,比较两组患者预后情况及并发症发生率。结果与对照组比较,观察组脑脊液转清时间及脑脊液压力恢复时间更短,每日引流量更多(P<0.05);观察组5例(16.67%)出现并发症,对照组13例(43.33%)出现并发症,差异有统计学意义(P<0.05)。结论针对合并外伤性蛛网膜下腔出血的颅脑损伤患者,采用颅内压监护下脑室外引流联合腰穿治疗可缩短脑脊液转清时间、脑脊液压力恢复时间,增加每日引流量,减少并发症,疗效显著。
Objective To explore the effect of external brain drainage combined with lumbar puncture under the monitoring of intracranial pressure in patients with traumatic subarachnoid hemorrhage.Methods 60 patients with traumatic subarachnoid hemorrhage in the period from February 2018 to February 2019 were included as the research subjects.The patients were divided into two groups by single and double lot drawing.The group with single lot drawing was included in the control group and the group with double lot drawing was included in the observation group,with 30 cases in each group.The control group was treated by conventional way,and the observation group was treated by brain outdoor drainage combined with lumbar puncture under the monitoring of intracranial pressure.The prognosis and complication rate of the two groups were compared.Results Compared with the control group,the time of cerebrospinal fluid clearance and the time of cerebrospinal fluid pressure recovery in the observation group were shorter,and the daily drainage volume was more(P<0.05);5 cases(16.67%)in the observation group had complications,13 cases(43.33%)in the control group had complications,the difference was significant(P<0.05).Conclusion For the patients with traumatic subarachnoid hemorrhage,the treatment of brain drainage combined with lumbar puncture under the monitoring of intracranial pressure can shorten the time of cerebrospinal fluid clearance,the time of cerebrospinal fluid pressure recovery,increase the daily drainage flow,reduce the complications and highlight the curative effect.
