摘要
目的:观察益气化瘀平肝滋肾方对高血压早期肾损害患者胱抑素C(CysC)、微量白蛋白(mAlb)、β2微球蛋白(β2-MG)及肾动脉血流动力学的影响。方法:115例高血压早期肾损害患者随机分为观察组(58例)和对照组(57例)。对照组给予常规西医治疗,观察组在对照组基础上加用益气化瘀平肝滋肾方治疗。连续治疗3个疗程后,比较两组治疗效果和药品不良反应发生情况,以及治疗前后中医证候积分、血压[收缩压(SBP)、舒张压(DBP]、肾损害指标(血清CysC、尿mAlb、β2-MG)、肾动脉血流动力学[舒张末期流速(EDV)、收缩期峰值流速(PSV)、肾动脉阻力指数(RI)]等指标变化。结果:观察组总有效率明显高于对照组(91.38%vs.73.68%,P<0.05)。治疗后,两组患者的中医证候积分,SBP、DBP,血清CysC、尿β2-MG、MAlb水平,肾动脉EDV、PSV、RI等各项指标均较治疗前显著改善(P<0.05);且观察组上述指标均明显优于对照组(P<0.01)。两组药品不良反应发生率差异无统计学意义(P>0.05)。结论:在常规西医治疗基础上益气化瘀平肝滋肾方治疗高血压早期肾损害,疗效确切,能有效减轻症状,改善血压、肾功能,调节肾动脉血流动力学,安全可靠。
Objective:To explore the effects of Yiqi Huayu Pinggan Zishen recipe on cystatin C(CysC),microalbumin(mAlb),β2 microglobulin(β2-MG)and renal artery hemodynamics in hypertensionpatients with early renal damage.Methods:A total of 115 hypertensionpatients with early renal impairment were treated in our hospital from August 2017 to December 2018 as the research objects,and according to the random number table method,they were divided into observation group(58 cases)and control group(57 cases).The control group was treated with the conventional western medicine,and the observation group was treated with Yiqi Huayu Pinggan Zishen recipe based on the control group.The therapeutic effects,TCM syndrome scores,blood pressure[systolic blood pressure(SBP),diastolic blood pressure(DBP)],renal damage indicators(serum CysC,urine mAlb,β2-MG),renal arterial hemodynamics[end-diastolic velocity(EDV),peak systolic velocity(PSV),renal artery resistance index(RI)]and adverse events were compared between the two groups.Results:The total effective rate in the observation group was higher than that in the control group(91.38%vs.73.68%,P<0.05).The scores of TCM syndromes,levels of SBP and DBP,and serum levels of CysC,urineβ2-MG and MAlb in the observation group were lower than those in the control group after 3 courses of treatment,the renal artery EDV and PSV in the observation group were higher than those in the control group,and the RI was lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the observation group and the control group(P>0.05).Conclusion:On the basis of conventional western medicine treatment,Yiqi Huayu Pinggan Zishen recipe is the first treatment method for early renal damage in hypertension with definite efficacy,which can effectively reduce symptoms,improve blood pressure and renal function,and regulate renal artery hemodynamics with promising safety and reliability.
