摘要
本文回顾性分析了1例哮喘合并嗜酸粒细胞肺炎、痰细菌培养可见真菌感染的66岁女性患者的临床特征和诊治经过。该患者有15年的支气管哮喘病史,本次以哮喘加重伴有嗜酸性粒细胞增多症和双侧肺炎入院,外周血嗜酸性粒细胞明显升高,骨髓涂片和血细胞涂片提示嗜酸性粒细胞增多症,痰液细菌培养可见白色念珠菌感染。嗜酸性肺病是一种少见的肺部疾病,哮喘合并嗜酸粒细胞肺炎的病例报道甚少,缺少治疗相关疾病的临床经验,本例患者经过抗生素、抗真菌药及糖皮质激素的相应治疗后好转出院。本文对此进行了回顾性分析,希望可以为临床诊治提供借鉴。
In this paper,the clinical characteristics,diagnosis and treatment of a woman aged 66 years with asthma complicated with eosinophilic pneumonia and fungal infection in sputum culture were analyzed retrospectively.The patient,who had a history of bronchial asthma for 15 years,was admitted to the hospital because of the aggravation of asthma with eosinophilia and bilateral pneumonia.The test results showed that the eosinophils in the peripheral blood increased significantly,the bone marrow smear and blood cell smear showed eosinophilia,and Candida albicans infection in sputum culture.Eosinophilic lung disease was a rare lung disease.Only a few cases of asthma with eosinophilic pneumonia had been reported,and there was no standard clinical experience in the treatment of related diseases.Therefore,we empirically treatment with antibiotics,antifungals and glucocorticoids,after the active treatment,the patient was discharged.We analyzed the situation of this patient retrospectively,expecting to provide reference for the diagnosis and treatment of related diseases in the future.
