两种假体治疗MasonⅢ型桡骨头骨折的疗效对比分析

摘要 目的探讨人工桡骨头置换术骨水泥型单极桡骨头假体与松配型桡骨头假体的中期疗效及功能对比。方法对2013年10月至2019年8月于某院接受骨水泥型单极桡骨头假体或者松配型桡骨头假体治疗MasonⅢ型桡骨头骨折患者进行随访研究。其中使用骨水泥型假体的患者共计33例,男21例,女12例,骨折部位:左侧14例,右侧19例;松配型假体患者27例,男18例,女9例,骨折部位:左侧12例,右侧15例,所有患者均为闭合骨折。结果所有患者均获得12~24个月随访,平均14.75个月,患者术后屈伸度:骨水泥组(128.03±2.23/5.82±2.72)°,松配组(129.11±2.19/5.07±2.60)°,两组屈伸方面差异无统计学意义,P>0.05;旋前旋后:骨水泥组(76.30±3.23/78.15±3.87)°,松配组(78.85±4.61/81.59±4.13)°,故松配组在旋转方面明显优于骨水泥组,P<0.05,随访12个月时改良Broberg-Morrey评分:骨水泥组(86.67±5.92)分,松配组(91.04±5.16)分,P<0.05;根据末次随访Broberg-Morrey评分标准分级,分级评定:骨水泥组优7例,良21例,可5例,差0例;松配组优10例,良15例,可2例,差0例,经Mann-Whitney检验,松配组组的疗效好于骨水泥组,差异有统计学意义(Z=-3.069,P<0.05)。结论松配型桡骨头假体较之骨水泥型单极假体有更好的治疗效果,功能及活动度方面有一定改善。 Objective To compare the mid-term curative effect and function of bone cement unipolar radial head prosthesis and loose fitting radial head prosthesis in artificial radial head replacement.Methods Patients with Mason Ⅲtype radial head fracture at a hospital from October 2013 to August 2019 who received bone cement unipolar radial head prosthesis or loose radial head prosthesis were followed up for study.Among them,there were 33 patients who used cemented prosthesis,including 21 males and 12 females.The fracture sites were 14 cases on the left side and 19 cases on the right side.27 cases were given loose fitting prosthesis,including 18 males and 9 females.The fracture sites were 12 cases on the left side and 15 cases on the right side.All patients were closed fractures.Results All patients were followed up for 12 to 24 months,with an average of 14.75 months.The postoperative flexion and extension of bone cement group and loose fitting group were(128.03±2.23/5.82±2.72)° and(129.11±2.19/5.07±2.60)°respectively(P>0.05).Therefore,there was no significant difference in flexion and extension between the two groups.Pronation and supination:the bone cement group(76.30±3.23/78.15±3.87)°,the loose fitting group(78.85±4.61/81.59±4.13)°,P<0.05 ttherefore,the loose fitting group was superior to the bone cement group in pronation and supination.The moditied Broberg-Morrey scores in the 12 months of follow-up:the bone cement group(86.67±5.92)points,the loose fitting group(91.04±5.16)points,P<0.05.According to the Broberg-Morrey scores in the last follow-up,7 cases were excellent,21 cases were good,5 cases were acceptable,and 0 case was poor in bone cement group,and 10 cases were excellent,15 cases were good,2 cases were acceptable,and 0 case was poor in loose fitting group.The results of Mann-Whitney test showed that the curative effect of loose fitting group was better than that of the bone cement group,and the difference was statistically significant(Z--3.069,P<0.05).Conclusion Loose fitting radial head prosthe
