摘要
目的采用三维高分辨率磁共振成像技术探讨大脑中动脉粥样硬化高危斑块的磁共振成像特征与缺血性事件复发的相关性。方法搜集首次发生缺血性事件的大脑中动脉粥样硬化性狭窄患者进行为期2年的随访,符合纳入及排除标准的患者连续入组本研究,根据规律内科治疗的情况下,大脑中动脉斑块导致的病变所在动脉供血区是否再次发生缺血性事件,将入选研究患者大脑中动脉斑块分为高危斑块组和低危斑块组,并均在3.0 T磁共振上行常规MRI扫描、3D快速自旋回波序列扫描,比较高危斑块组和低危斑块组患者的临床资料(年龄、性别、吸烟史、高血压史、糖尿病史、高脂血症)和斑块成像特征(管腔狭窄率、重构指数、偏心指数、强化程度)。结果共计50例符合纳入和排除标准的患者入选本研究,其中高危斑块22例,低危斑块28例,高危斑块组和低危斑块组患者的临床资料(年龄、性别、吸烟史、高血压史、糖尿病史、高脂血症)差异均无统计学意义(P>0.05)。斑块成像特征中高危斑块组的重构指数、偏心指数和强化程度均高于低危斑块组(P<0.05)。结论三维高分辨率磁共振成像技术检测可以作为识别高危斑块的良好工具,帮助评估缺血性事件复发风险。
Objective To analyze the relations between middle cerebral artery high-risk plaques and recurrent ischemic events by 3 D turbo spin echo sequence.Methods A 2-year follow-up of middle cerebral artery atherosclerotic patients who had an ischemic event occurred for the first time was collected.Patients who met the inclusion and exclusion criteria were continuously enrolled in this study.The disease-causing plaques of the middle cerebral artery were divided into high-risk plaque group and low-risk plaque group according to whether ischemic events recur in the area of blood supply or not.The risk factors of ischemic recurrence(age,sex,smoking history,hypertension history,diabetes history,total cholesterol)and plaque imaging characteristics(Cavity stenosis rate,reconstruction index,eccentricity index,enhancement degree)were observed.Results A total of 50 patients who met the inclusion and exclusion criteria were enrolled in this study.There were no significant differences in risk factors of ischemic recurrence(age,sex,smoking history,hypertension history,diabetes history,total cholesterol)between high-risk plaque group and low-risk plaque group(P>0.05).The reconstruction index,eccentricity index and enhancement degree of high-risk plaque group were higher than those of low-risk plaque group(P<0.05).Conclusion 3 D fast spin echo sequence detection can be used as a good tool to identify high-risk plaques and help to assess the risk of recurrence of ischemic events.
