摘要
目的分析聚焦超声联合抗人乳头瘤病毒(HPV)β-葡聚糖功能敷料治疗宫颈HPV感染的疗效及免疫功能变化。方法选取2017年11月~2019年11月我院收治的96例宫颈拭子核酸分子杂交法检测HPV阳性高危型患者,将所有患者随机分为联合治疗组、单一治疗组以及单纯对照组,每组各32例。单一治疗组患者给予抗HPVβ-葡聚糖功能敷料进行治疗,联合治疗组患者给予超声聚焦联合抗HPVβ-葡聚糖功能敷料进行治疗,单纯对照组仅行门诊随访。比较三组患者临床治疗效果、Th1/Th2细胞因子水平以及不良反应发生情况。结果联合治疗组患者治疗后总有效率高于单一治疗组(P<0.05)。联合治疗组、单一治疗组患者躯体角色、躯体功能、机体疼痛、整体健康、活力、心理卫生、社会功能以及情绪角色等生活质量评分均较治疗前显著上升,且联合治疗组各维度评分均明显高于单一治疗组,单一治疗组各维度评分均明显高于单纯对照组(P<0.05)。γ-干扰素(IFN-γ)、白细胞介素-2(IL-2)水平与治疗前相比升高,且联合治疗组明显高于单一治疗组,单一治疗组明显高于单纯对照组;而白细胞介素-4(IL-4)、白细胞介素-6(IL-6)水平则与治疗前相比降低,且联合治疗组显著低于单一治疗组,单一治疗组显著低于单纯对照组(P<0.05)。联合治疗组3例患者在术后7 d发生创面脱痂而导致阴道血流量增加,不良反应发生率为9.38%,但3组不良反应发生情况比较差异无统计学意义(P>0.05)。结论聚焦超声联合抗HPVβ-葡聚糖功能敷料治疗宫颈HPV感染临床效果显著,转阴率较高,可有效改善细胞免疫水平,提高患者生活质量,减少不良反应,可在临床推广应用。
Objective To study the short-term efficacy and immune function changes of focused ultrasound combined with anti HPVβ-glucan functional dressing in the treatment of cervical HPV persistent infection.Methods 96 patients who using prospective research methods admitted in our hospital from November 2017 to November 2019 were selected for cervical swab nucleic hybridization to detect HPV positive high-risk type.The patients were divided into the combined treatment group,the single treatment group and the single control group,32 cases in each group.Patients in the single treatment group were treated with anti-HPVβ-glucan functional dressings.Patients in the combined treatment group were given ultrasound focusing combined with anti-HPVβ-glucan functional dressings.Patients in the simple control group were only followed up in the clinic.The clinical effects,changes in Th1/Th2 cytokine levels,and adverse reactions were observed and compared.Results The total effective rate of combined treatment group was higher than that of single treatment group(P<0.05).The scores of body role,body function,body pain,overall health,vitality,mental health,social function and emotional role in the combined treatment group and the single treatment group were significantly higher than those before treatment,and the scores of each dimension in the combined treatment group were significantly higher than those in the single treatment group,and the scores in the single treatment group were significantly higher than those in the control group(P<0.05).The levels of interferon-γ(IFN-γ)and interleukin-2(IL-2)in the combined treatment group were significantly higher than those in the single treatment group and the control group,while the levels of IL-4 and IL-6 in the combined treatment group were significantly lower than those in the single treatment group In the treatment group,the single treatment group was significantly lower than the control group(P<0.05).The incidence of adverse reactions was 9.38%,but there was no significant difference b
