摘要
目的探讨螺旋CT与核磁共振在肝脏囊性占位疾病诊断中的临床价值。方法随机该院2018年3月-2019年10月疑似肝脏囊性占位患者43例,术前分别进行螺旋CT检查、核磁共振检查,参照术后病理检查结果,通过诊断符合率、敏感性、特异性、漏诊率等指标,评估螺旋CT、核磁共振的临床诊断应用价值。结果以术后病理检查结果为金标准,发现核磁共振检查的诊断符合率93.02%、特异性95.65%、敏感性90.00%均高于螺旋CT检查67.44%、73.91%、60.00%,漏诊率10.00%低于螺旋CT检查40.00%,对比差异有统计学意义(χ^2=8.871、4.212、4.800、4.800,P<0.05);螺旋CT、核磁共振诊断肿瘤囊变与炎症性囊性病变的影像学特征表现有差异。结论对肝脏囊性占位疾病患者分别予以螺旋CT检查、核磁共振检查,均可取得异性的诊断效果,但核磁共振检查的诊断符合率、敏感性、特异性较优,且漏诊率低,同时可以清晰显示病理特征表现。
Objective To explore the clinical value of spiral CT and MRI in the diagnosis of hepatic cystic disease.Methods Randomly selected 43 patients with suspected liver cystic space occupying in the hospital from March 2018 to October 2019.Spiral CT examination and MRI examination were performed before operation,and the results of postoperative pathological examination were followed to pass the diagnosis coincidence rate and sensitivity,specificity,missed diagnosis rate and other indicators to evaluate the clinical diagnostic value of spiral CT and MRI.Results Taking the postoperative pathological examination results as the gold standard,it was found that the diagnostic coincidence rate93.02%,specificity 95.65%and sensitivity 90.00%of MRI examination were higher than spiral CT examination 67.44%,73.91%,60.00%,the missed diagnosis rate 10.00%,lower than spiral CT examination 40.00%,the contrast difference was significant(χ^2=8.871,4.212,4.800,4.800,P<0.05);spiral CT,MRI diagnosis of tumor cystic lesions and inflammatory cystic lesions had different imaging features.Conclusion Spiral CT examination and MRI examination for patients with hepatic cystic space-occupying disease can obtain the diagnosis effect of the opposite sex.However,the diagnostic coincidence rate,sensitivity and specificity of MRI examination are better,and the missed diagnosis rate is low,which can clearly show the pathological characteristics.
