摘要
目的探析脾氨肽对巨细胞病毒(CMV)感染儿童CD4+、CD8+及免疫球蛋白的影响。方法收集徐州医科大学附属徐州儿童医院2018年2月-2019年5月收治的CMV患儿106例,采用单盲法随机分为对照组(n=53)和观察组(n=53)。对照组采取更昔洛韦治疗,观察组采用脾氨肽+更昔洛韦治疗,持续治疗4周,检测两组尿CMV-DNA载量,CD4+、CD8+、CD4+/CD8+、IgG、IgA、IgM水平。结果治疗14、28 d后,两组尿CMV-DNA载量均明显降低,与治疗前比较差异有统计学意义(P<0.05);观察组治疗14、28 d尿CMV-DNA载量降低水平显著优于对照组,差异有统计学意义(P<0.05)。两组治疗28 d后,CD4+、CD4+/CD8+均明显升高,CD8+明显降低,与治疗前比较差异有统计学意义(P<0.05),且观察组CD4+、CD4+/CD8+升高幅度与CD8+降低幅度均明显优于对照组,差异有统计学意义(P<0.05)。观察组治疗28 d后IgG、IgA、IgM均明显升高,与治疗前比较差异有统计学意义(P<0.05);对照组治疗前后IgG、IgA、IgM比较,差异无统计学意义(P>0.05);观察组IgG、IgA、IgM改善效果明显优于对照组,差异有统计学意义(P<0.05)。结论脾氨肽可有效调节T淋巴细胞亚群CD4+、CD8+水平,增强免疫球蛋白IgG、IgA、IgM浓度,缓解CMV合并症反应,调节自主免疫平衡。
Objective To investigate the effect of spleen ammonia peptide on CD4+T cells,CD8+T cells and immunoglobulin in children with cytomegalovirus(CMV) infection.Methods A total of 106 children with CMV treated in our hospital from February 2018 to May 2019 were divided into groups by single-blind random method.The control group was treated with ganciclovir,and the observation group was treated with spleen aminonia peptide+ganxi Lowe treatment continued for 4 weeks.The urine CMV-DNA load,CD4+T cells,CD8+T cells,CD4+/CD8+ ratio,IgG,IgA,and IgM levels were measured in both groups.Results After 14 days and 28 days of treatment,the urine CMV-DNA load of the two groups was significantly reduced,and there was a statistically significant difference compared with before treatment(P<0.05).The observation group had a significantly lower level of urine CMV-DNA load after 14 days and 28 days of treatment.There was statistical significance in the control group(P<0.05).After 28 days of treatment in both groups,CD4+T cells,CD4+/CD8+increased significantly,CD8+T cells significantly decreased,and there was a statistically significant difference compared with before treatment(P<0.05),and the increase of CD4+T cells,CD4+/CD8+ and decrease of CD8+T cells in the observation group were more obvious than those of control group(P<0.05).In the observation group,IgG,IgA,and IgM levels were significantly increased after 28 days of treatment,and the differences were statistically significant compared with those of before treatment(P<0.05).In the control group,IgG,IgA,and IgM levels before and after treatment were not statistically significant(P>0.05).The improvement of IgG,IgA,IgM levels in the observation group was significantly better than those in the control group(P<0.05).Conclusion Spleen ammonia peptide could effectively regulate CD4+T cells,CD8+T cells subsets,enhance immunoglobulin IgG,IgA,IgM concentrations,alleviate CMV comorbidity response and regulate autonomic immune balance.
