64排螺旋CT三维重建技术诊断主动脉夹层的应用价值 被引量:2

The application value of 64 slice spiral CT three-dimensional reconstruction technique in the diagnosis of aortic dissection

摘要 目的:探析应用64排螺旋CT常规、多种后处理手段检查主动脉夹层的临床诊断效果。方法:选取2006年1月-2019年7月诊疗的主动脉夹层患者145例,所有患者均接受64排螺旋CT平扫、增强扫描,将所得图像应用后处理工作站进行重建,包括多平面重建(MPR)、曲面重建(CPR)、最大密度投影(MIP)、容积再现重建(VR)。探析64排螺旋CT常规与多种后处理技术联合对于主动脉夹层的临床诊断价值。结果:145例患者接受DeBakey分型,其中Ⅰ型34例,Ⅱ型4例,Ⅲ型107例。应用64层螺旋CT诊断主动脉夹层的特异度与敏感度均达到100.0%;对破口、真假腔、内膜瓣应用MPR和CPR的显示率对应为95.9%及100.0%;对破口应用MIP检测显示率为14.5%;对破口应用VR检测显示率为16.6%;对真假腔和内膜瓣应用MIP、VR的显示率对应为69.7%及94.5%。结论:应用64排螺旋CT多种后处理手段中,对于破口、真假腔以及内膜瓣显示方面,MPR与CPR相对于MIP、VR有显著优势,对主动脉夹层在进行三维重建过程中首选MPR与VR技术,MIP可放弃不用。 Objective:To explore and analyze the clinical diagnosis effect of 64 slice spiral CT and various post-processing treatment methods in the examination of aortic dissection.Methods:From January 2006 to July 2019,145 patients with aortic dissection were selected.All patients received 64 slice spiral CT plain scan and enhanced scan,and the obtained images were reconstructed by post-processing workstation,including multiplanar reconstruction(MPR),curve planar reconstruction(CPR),maximal intensity projection(MIP)and volume reconstruction(VR).To explore and analyze the clinical value of 64 slice spiral CT combined with various post-processing techniques in the diagnosis of aortic dissection.Results:145 cases of patients accepted DeBakey typing,34 of them were type I,4 of them were type II,107 of them were type III.The specificity and sensitivity of 64 slice spiral CT in the diagnosis of aortic dissection reached 100.0%.The display rate of MPR and CPR were 95.9%and 100.0%respectively for the diagnosis of rupture,true and false lumen and intimal flap.The display rate of MIP detection on the rupture was14.5%.The display rate of VR detection for rupture was 16.6%.The display rates of MIP and VR for true and false lumen and intimal flap were 69.7%and 94.5%respectively.Conclusion:MPR and CPR have significant advantages over MIP and VR in the application of various post-processing treatment methods of 64 slice spiral CT,and MPR and VR are preferred in the three-dimensional reconstruction of aortic dissection,while MIP can be abandoned.
