摘要
目的:评估小剂量腰麻联合超声引导下髂筋膜阻滞应用于老年股骨手术中的效果。方法:2019年1月-2020年5月收治行股骨手术的老年患者60例,随机分为两组,各30例。试验组采用小剂量腰麻联合超声引导下髂筋膜阻滞的麻醉方式;对照组采用持续性硬膜下阻滞的麻醉方式。比较两组应用效果。结果:试验组整体麻醉效果优于对照组,差异有统计学意义(P<0.05);试验组术中恶心呕吐发生率低于对照组,差异有统计学意义(P<0.05);试验组麻黄碱使用率明显低于对照组,差异有统计学意义(P<0.05);试验组术后各时间点疼痛程度均低于对照组,差异有统计学意义(P<0.05)。结论:小剂量腰麻联合超声引导下髂筋膜阻滞的麻醉方式,操作简单、麻醉效果满意、循环干扰小、术后镇痛效果佳,有效保障患者围术期安全。
Objective:To evaluate the effect of low-dose spinal anesthesia combined with ultrasound-guided iliac fascia block in elderly patients who undergoing femoral surgery.Methods:From January 2019 to May 2020,60 cases of elderly patients who underwent femoral surgery were enrolled,they were randomly divided into two groups with 30 cases in each group.The experimental group used low-dose spinal anesthesia combined with ultrasound-guided iliac fascia block,while the control group used continuous subdural block.The application effects of the two groups was compared.Results:The overall anesthetic effect of the experimental group was better than that of the control group,the difference was statistically significant(P<0.05).The incidence rates of nausea and vomiting in the experimental group were lower than that in the control group,the differences were statistically significant(P<0.05).The utilization rate of ephedrine in the experimental group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).After surgery,the degree of pain of each period in the experimental group were lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Low-dose spinal anesthesia combined with ultrasound-guided iliac fascia block anesthesia has simple operation,satisfactory anesthesia effect,low circulatory interference and good postoperative analgesia,which can effectively guarantee patients’safety during the perioperative period.
