无创序贯性机械通气与肺复张联合治疗ARDS的疗效分析

摘要 目的分析无创序贯性机械通气与肺复张联合治疗方法在ARDS(急性呼吸窘迫综合征)方面的应用效果。方法研究对象选择2019年1—12月期间内该院收治的80例ARDS患者,将其按照数字随机法划分为观察组和对照组,每组40例。对照组选择有创序贯性机械通气治疗,观察组采取无创序贯性机械通气配合肺复张方法治疗。比较两组患者治疗后的临床疗效和血气指标变化情况,判断治疗效果。结果观察组的整体疗效达到了95.0%远高于对照组的77.5%,差异有统计学意义(χ^2=5.165,P<0.05);在血气分析数据上,观察组的PaO_2/FiO_2(290.0±11.6)mmHg、PaO_2(85.9±6.8)mmHg、PaCO_2(55.0±5.7)mmHg和SaO_2(91.0±4.6)%4项指标均要显著优于对照组的(261.8±11.8)mmHg、(74.2±7.1)mmHg、(72.7±5.3)mmHg和(81.1±5.1)%,证实了观察组患者的氧合状况改善良好,数据结果差异有统计学意义(t=3.305、3.393、3.567、3.402,P<0.05)。结论无创序贯性机械通气和肺复张联合应用在ARDS患者的治疗过程中可以发挥显著作用,在降低血流动力学影响的同时改善呼吸氧合,可以作为今后的主要治疗手段。 Objective To analyze the effect of the combination of noninvasive sequential mechanical ventilation and atelectasis on ARDS(acute respiratory distress syndrome).Methods 80 patients with ARDS admitted to the hospital from January to December 2019 were divided into observation group and control group with 40 cases in each group.The control group was treated with invasive sequential mechanical ventilation,while the observation group was treated with noninvasive sequential mechanical ventilation combined with pulmonary reexpansion.After treatment,the clinical effect and blood gas index of the two groups were compared.Results The overall efficacy of the observation group reached 92.5%,which was much higher than that of the control group(77.5%).the difference was statistically significant(χ2=5.165,P<0.05).The results were statistically significant.In the blood gas analysis data,PaO2/FiO2(290.0±11.6)mmHg,PaO2(85.9±6.8)mmHg,PaCO2(55.0±5.7)mmHg,SaO2(91.0±4.6)%,the four indexes of the observation group were significantly better than those of the control group(261.8±11.8)mmHg,(74.2±7.1)mmHg,(72.7±5.3)mmHg and(81.1±5.1)%respectively,which confirmed that the patients in the observation group had a good oxygenation,and the difference was statistically significant(t=3.305,3.393,3.567,3.402,P<0.05).Conclusion The combination of noninvasive sequential mechanical ventilation and pulmonary reexpansion can play a significant role in the treatment of ARDS.It can reduce the hemodynamic effect and improve respiratory oxygenation,which can be used as the main treatment in the future.
