摘要
目的:探讨乳腺癌各分子病理分型在数字乳腺三维断层摄影(DBT)X线表现的特征。方法:回顾性分析经手术和病理证实的乳腺癌108例(110个病灶),根据分子病理特征,Luminal A型11个,Luminal B1型49个,Luminal B2型19个,HER-2过度表达型12个,三阴性型19个。根据DBT X线不同表现,采用χ2检验和Fisher确切概率法分析各分子亚型在各X线表现间分布差异。结果:结果显示病灶是否单纯性肿块、恶性钙化、毛刺征及病灶大小在Luminal A、Luminal B1、Luminal B2、HER-2过表达及三阴性分子亚型间存在统计学差异(χ2=14.523,14.523,Fisher=10.548,9.699,P<0.05)。而肿块(伴或不伴钙化)、病灶边缘清或模糊、病灶边缘是否纠集及淋巴结是否转移在各分子亚型间无明显统计学差异。结论:乳腺癌各分子亚型部分DBT X线表现具备一定特征性,对乳腺癌分子病理分型有预测价值。
Objective: To investigate the value of using digital breast tomosysthesis(DBT) to predict the molecular subtypes of breast carcinoma. Methods: This article reviewed 108 patients with surgically and pathologically verifies breast carcinoma,and we get 110 lesions at all. Among the lesions, there are 11 lesions with Luminal A, 49 lesions with Luminal B1, 19 le-sions with Luminal B2, 12 lesions with HER-2 over-expressed, and 19 lesions with triple negative. According to the express of DBT, the data was anaysed by Fisher’s exact test or Chi-square test. Results: The Fisher’s exact test or Chi-square test among multiple groups indicated that there were statistical differences with simple tumor, calcification, spicules of margin and tumor size in five molecular subtypes(χ~2=14.523, 14.523, Fisher=10.548, 9.699, P<0.05). Conclusion: The performance of DBT can be applied to prove some evidence for the dissection of molecular subtypes of breast carcinoma.
