摘要
目的:探究超声三维斑点追踪成像技术(3D-STI)在风湿性心脏病诊断及预测评估中的应用。方法:将86例医院收治的风湿性二尖瓣狭窄(MS)患者纳入观察组,将同期于医院体检的40名健康体检者纳入对照组,采集所有受检者的临床资料,并进行3D-STI等检查,收集3D-s TI相关数据。对比两组受检者3D-STI左心房前后径(LAD)及二尖瓣舒张期峰值流速(MVV(MAX))的差异,采用Spearman分析整体径向峰值应变(GRPS)、整体圆周峰值应变(GCPS)、整体纵向峰值应变(GLPS)及整体面积峰值应变(GAPS)与左心房主动排空分数(LAAEF)的关系。结果:观察组患者的LAD及MVV(MAX)高于对照组,二尖瓣口面积(MVA)低于对照组(t=18.785,t=19.082,t=61.844;P<0.01);观察组患者的左心房总排空分数(LATEF)、左心房被动排空分数(LAPEF)、左心房被动排空容积(LAVIp)及LAAEF水平低于对照组,左心房最大容积指数(LAVI(max))水平高于对照组(t=17.821,t=22.544,t=22.544,t=10.333,t=10.490;P<0.01);观察组患者左心房16节段全部APS、大部分LPS及CPS水平均低于对照组。观察组患者的GRPS、GCPS、GLPS及GAPS水平低于对照组(t=2.587,t=15.417,t=25.554,t=23.096;P<0.05)。GRPS、GCPS、GLPS及GAPS与LAAEF均呈正相关(r=0.517,r=0.358,r=0.562,r=0.618;P<0.05)。结论:MS患者左心房的储存能力、管道功能及助力泵功能减退,GCPS、GLPS及GAPS水平降低,且与LAAEF呈明显正相关,3D-STI可检测MS患者左心房结构及心肌功能的变化,并作为评估诊断MS的检验方法。
Objective:To explore the application of ultrasound three-dimensional speckle tracking imaging(3D-STI)technique in the diagnosis,prediction and assessment of rheumatic heart disease.Methods:86 patients with rheumatic mitral stenosis(MS)who admitted to hospital were included in observation group,and 40 healthy people who were confirmed by physical examination in the same period were included in control group.The clinical data of all subjects were collected,and all of persons underwent 3D-STI examination.And then,the 3D–STI relevant data were collected.The differences of left atrial diameter(LAD)and peak diastolic velocity of mitral valve(MVVmax)in 3D-STI between two groups were compared.Spearman was used to analyze the relationships between GRPS,GCPS,GLPS,GAPS and LAAEF,respectively.Results:The LAD and MVVmax of observation group were significantly higher than those of control group,and the mitral valve area(MVA)of observation group was significantly lower than that of control group(t=18.785,t=19.082,t=61.844,P<0.01).The levels of LATEF,LAPEF,LAVIp and LAAEF of observation group were significantly lower than those of control group,and the LAVImax level of observation group was significantly higher than that of control group(t=17.821,t=22.544,t=22.544,t=10.333,t=10.490,P<0.01).The levels of APS,the most of LPS and the entire APS of the 16 segments of left atrium of observation group were lower than those of control group.And the levels of GRPS,GCPS,GLPS and GAPS of observation group were significantly lower than those of control group(t=2.587,t=15.417,t=25.554,t=23.096,P<0.05).GRPS,GCPS,GLPS and GAPS were positively correlated with LAAEF(r=0.517,r=0.358,r=0.562,r=0.618,P<0.05),respectively.Conclusion:In MS patients,the storage capacity,pipeline function and booster pump function of left atria decrease,and the levels of GCPS,GLPS and GAPS decline,and these levels significantly positively correlate with LAAEF.3D-STI can detect the changes of left atrial structure and myocardial function in MS patients,and can be use
