摘要
【目的】观察头皮针刺联合镜像疗法治疗卒中后单侧忽略的临床疗效。【方法】将100例卒中后单侧忽略患者随机分为头皮针刺组(头针组)33例、镜像治疗组(镜像组)33例,头皮针联合镜像治疗组(联合组)34例。头针组给予头皮针刺治疗;镜像组给予镜像疗法治疗;联合组在头皮针刺治疗的基础上,联合镜像疗法进行治疗。3组患者均每天治疗2次,每周治疗6 d,连续治疗6周。治疗6周后,观察3组患者治疗前后单侧忽略行为观测量表(CBS)总分和行为注意障碍评测量表(BIT)总分的变化情况,并比较3组患者简易智能状态检查量表(MMSE)评分和改良Barthel指数量表(MBI)评分的变化情况。【结果】(1)治疗后,3组患者CBS量表总分和BIT量表总分均明显改善(P<0.05),且联合组在改善CBS量表总分和BIT量表总分方面均明显优于头针组和镜像组,差异均有统计学意义(P<0.05)。(2)治疗后,3组患者的MMSE评分均明显改善(P<0.05),且联合组在改善MMSE评分方面均明显优于头针组和镜像组,差异均有统计学意义(P<0.05)。(3)治疗后,3组患者的MBI评分均明显改善(P<0.05),且联合组在改善MBI评分方面均明显优于头针组和镜像组,差异有统计学意义(P<0.05)。【结论】头皮针刺联合镜像疗法治疗卒中后单侧忽略,能够明显地改善患者单侧忽略的临床症状,提高患者的认知功能和日常生活能力,疗效显著。
Objective To observe the curative efficacy of scalp acupuncture combined with mirror therapy for the treatment of poststroke unilateral neglect.Methods One hundred patients with poststroke unilateral neglect were randomly divided into scalp acupuncture group(33 cases),mirror group(33 cases),combination group(34 cases).The scalp acupuncture group received scalp acupuncture treatment,mirror group received mirror therapy treatment,and combination group received scalp acupuncture combined with mirror treatment,two times per day,6 days per week,the treatment lasting 6 weeks.After 6 weeks of treatment,the changes in total scores of unilateral neglect Catherine-Bergego Scale(CBS)and Behavioural Inattention Test(BIT)were observed in the 3 groups before and after treatment,and the scores of Mini-mental State Examination(MMSE)and Modified Barthel Index(MBI)were compared in the 3 groups.Results(1)After treatment,the total scores of CBS and BIT in the 3 groups were improved(P<0.05),and the improvement in total scores of CBS and BIT of the combination group was superior to that of the scalp acupuncture group and mirror group(P<0.05).(2)After treatment,the MMSE scores in the 3 groups were improved(P<0.05),and the improvement in MMSE scores of the combination group was superior to that of the scalp acupuncture group and mirror group(P<0.05).(3)After treatment,the MBI scores in the 3 groups were improved(P<0.05),and the improvement in MBI scores of the combination group was superior to that of the scalp acupuncture group and mirror group(P<0.05).Conclusion Scalp acupuncture combined with mirror therapy is effective for improving unilateral neglect symptoms in patients with poststroke unilateral neglect,and promoting cognitive functions and ability of daily life.
