多声波超洁净系统GentleWave在根管治疗中的研究进展 被引量:5

摘要 牙髓病和根尖周病多是由细菌感染引起的疾病,根管治疗术是主要的治疗手段。其中对根管系统进行机械化学联合预备,彻底控制根管内感染是根管治疗的重要环节。传统根管治疗术常常需要借助预备器械来清除根管内的感染物,然而过多的切削根管壁可造成牙体抗折性能的降低,增加根管偏移、穿孔和器械折断等风险。另外,由于根管系统的复杂性,难以彻底清除根管内的感染物质,从而导致治疗失败。因此,如何尽可能的保存牙体组织,同时提高治疗的效果,是目前研究的重点。最近,多声波超洁净系统GentleWave(GW)的问世,相比传统的机械化学预备,简化临床操作,避免了对根管的切削,在保存牙体组织的同时又提高了根管系统清理效果。本文将对此系统的原理及操作进行简要的介绍,并对其在根管治疗中的研究进展和应用的优势与不足进行评述。 Pulp and periapical diseases are often caused by bacterial infections,and root canal therapy is the main treatment method.The mechanochemical preparation for the root canal system is a necessary process to control infection completely.Endodontic instruments are employed in the traditional process for the removal of infection.However,excessive cutting of the root canal wall may result in the reduction of the tooth's flexural performance,and increase the risk of root canal migration and perforation and instrument fracture.In addition,the treatment may fails because of the difficulty of removing infection thoroughly determined by the complexity of root canal system.Therefore,it is the focus about how to improve the effectiveness of treatment,as well as preserving hard tissue as much as possible.Recently,GentleWave(GW),a multi-acoustic ultra-clean system,was introduced.Compared with traditional mechanochemical preparation,it simplifies clinical operation,avoids the cutting of root canal,and improves the cleaning effect of root canal system while preserving tooth tissue.In this paper,the principle and operation of this system are briefly introduced,and research progress of it in root canal therapy is reviewed,as well as its advantages and disadvantages.
