摘要
血管麻痹综合征(vasoplegic syndrome,VS)广泛存在于各种休克晚期以及心血管等大手术后,其导致的持续性低血压是患者死亡的重要原因。亚甲蓝(methylene blue,MB)可用于充分扩容和应用大剂量血管活性药物后仍然无法改善的VS患者,在维持VS相关的血流动力学稳定中发挥至关重要的作用。但目前MB在VS的应用仍存在较大争议。文章对MB在治疗不同病因所致的VS中的疗效和安全性进行综述,介绍其作用机制、用法、用量、副作用、适应证等特性,以期为临床工作中MB的合理使用提供依据。
Vasoplegic syndrome(VS)commonly appears in all kinds of late shock and cardiac surgery and cause continuous hypotension as an important reason of death of patients.Methylene blue(MB)can be used in VS patients who cannot be improved after volume enlargement and treatment with large doses of vasoactive drugs,and play an essential role in maintaining VS-related stable hemodynamics.However,there are currently a large number of disputes relating to its application in VS.This article is designed to review the effectiveness and safety of MB in the treatment of VS due to various causes.Furthermore,its mechanism of action,dose and dosing,side effects and implications were introduced,so as to proved evidence for rational use of methylene blue during clinical practice.
