摘要
目的探讨糖皮质激素不同给药方式治疗重症甲状腺相关眼病的临床效果。方法回顾性分析2018年1月至2019年10月我院的80例重症甲状腺相关眼病患者的临床资料,依据糖皮质激素给药方式将其分为冲击疗法组(40例,糖皮质激素冲击疗法)和口服治疗组(40例,糖皮质激素口服疗法)。比较两组患者的突眼度、软组织炎症评分、临床活动度评分(CAS)、临床疗效、全身副反应发生情况。结果治疗后,两组的突眼度、软组织炎症评分、CAS评分均较治疗前降低,且冲击疗法组低于口服治疗组(P<0.05)。冲击疗法组的治疗总有效率为90.0%,高于口服治疗组的65.0%(P<0.05)。冲击疗法组的全身副反应总发生率为20.0%,低于口服治疗组的42.5%(P<0.05)。结论糖皮质激素冲击疗法治疗重症甲状腺相关眼病的效果较口服治疗好,值得在临床推广。
Objective To explore the clinical effect of different administration methods of glucocorticoid in the treatment of severe thyroid-associated ophthalmopathy.Methods The clinical data of 80 patients with severe thyroid-associated ophthalmopathy in our hospital from January 2018 to October 2019 were retrospectively analyzed.According to the glucocorticoid administration method,the patients were divided into impact therapy group(40 cases,glucocorticoid impact therapy)and oral treatment group(40 cases,oral glucocorticoid therapy).The degree of exophthalmos,soft tissue inflammation score,clinical activity score(CAS),clinical efficacy and systemic side effects were compared between the two groups.Results After treatment,the degree of exophthalmos,soft tissue inflammation score and CAS score of the two groups were lower than those before treatment,and those in the impact therapy group were lower than the oral treatment group(P<0.05).The total effective rate of treatment of the impact therapy group was 90.0%,which was higher than 65.0%of the oral treatment group(P<0.05).The total incidence of systemic side effects in the impact therapy group was 20.0%,which was lower than42.5%in the oral treatment group(P<0.05).Conclusion Compared with oral glucocorticoid therapy,glucocorticoid impact therapy in the treatment of severe thyroid-associated ophthalmopathy has better effect,which is worthy of clinical promotion.
