摘要
目的探讨重症监护室(ICU)老年患者肠内营养(EN)相关性腹泻的影响因素,并制定针对性干预措施。方法选择2017年1月至2019年5月我院接受EN治疗的ICU患者102例作为研究对象,收集患者相关资料,分析其肠内营养期间腹泻的发生情况及相关影响因素。结果102例ICU患者中58例EN期间发生腹泻,发生率56.86%,其中51例(87.93%)患者腹泻发生在EN后7 d内,46例(79.31%)患者持续腹泻时间1~3 d。经多因素logistic回归分析显示,EN前禁食、EN制剂输注速度、EN制剂日用量、EN制剂温度、口服钾制剂和抗生素使用时间为影响ICU老年患者EN期间腹泻的危险因素。结论ICU老年患者EN期间腹泻发生率较高,且影响因素较多,应针对性做好肠内营养护理,以消除或降低腹泻的发生风险。
Objective To explore the influencing factors of enteral nutrition(EN)-related diarrhea in elderly patients in intensive care unit(ICU),and formulate targeted intervention measures.Methods We selected 102 cases of patients in ICU who were treated with EN in our hospital from January 2017 to May 2019 as the study objects,and the relevant data of the patients were collected and the incidence of diarrhea during enteral nutrition and related influencing factors were analyzed.Results Among 102 cases of ICU patients,58 cases of diarrhea occurred during EN,the incidence was 56.86%,51 cases(87.93%)of diarrhea occurred within 7 days after EN,and 46 cases(79.31%)continued diarrhea for 1 to 3 days.Multivariate logistic regression analysis showed that the fasting before EN,infusion rate of EN preparation,daily dosage of EN preparation,EN preparation temperature,oral potassium preparation and antibiotic using time were the risk factors that affected diarrhea in elderly patients in ICU during EN.Conclusion The incidence rate of diarrhea in elderly patients in ICU during EN was higher,and there were many influencing factors.So the enteral nutrition care should be done in order to eliminate or reduce the risk of diarrhea.
