摘要
总结探讨姜良铎教授运用从状态辨治“湿毒疫”的方法诊疗新型冠状病毒肺炎(COVID-19)的理论及经验。探讨疫毒损伤正气、气不摄津、化生湿浊、湿毒化热、痰瘀凝滞、阻络痹肺等基本病机以及疫毒循三焦膜系袭染他脏的转化病机特点,结合患者胸部CT辨识疫毒盛衰、肺部实阻程度,结合外周淋巴细胞辨识肺卫功能强弱,结合氧合指数辨识宗气盛衰,结合舌脉辨识病情进退,结合其他脏腑传变的临床表现和相关辅助检查辨识脏腑功能,综合分析患者当前病机,总结当前状态并施治,注重时时固护正气、扶正祛邪,才能及时截断病势取效。对于无症状核酸检测阳性患者,可从邪弱正盛或正虚邪恋的状态来辨治。从状态辨治开拓了新型冠状病毒肺炎中医药诊疗的思路和方法。
To discuss the theory and to summarize the experience of Professor JIANG Liangduo’s diagnosis of Corona Virus Disease 2019(COVID-19)as a dampness toxic plague and treatment of COVID-19 based on state Chinese medicine.The specific methods including the following aspects are taken into discussion.Take vital qi damage resulting from plague toxin,qi dysfunction in body fluid control and due to generation of dampness and turbidity,accumulation of dampness-toxin-heat,phlegm and blood stasis,obstruction in the collaterals and channel blockage in lungs as the basic pathogenesis features.Attach importance to the infection of other organs by plague toxin through triple energizer in transmission of disease.Combine chest CT showing the strength of plague toxin and obstruction level in lungs,peripheral lymphocytes showing the function of lung defensive qi,oxygenation index showing the strength of pectoral qi,tongue and pulse presentations showing progression of the disease,and clinical features or physical indexes or chemical indexes of other viscera syndromes in transmission of disease showing the functions of zang organs and fu organs all together into the assessment of current pathogenesis features.Use comprehensive treatment of TCM according to the specific patient’s current state of disease.Strongly protecting the vital qi from time to time,strengthening the body immunity resistance and eliminating pathogenic factors are the ways to cut off the changing trends during further process.While the way to cure the patients with no symptoms and the results remaining positive in nucleic acid test needs consideration from the perspective of pathogenic factors being weak but vital qi excessive,or vital qi being weak while pathogenic factors lingering.Therefore,the experience of state Chinese medicine theory and treatment in COVID-19 provides new ideas and methods in the area of TCM prevention and treatment of this new kind of infectious diseases.
