在闭合复位髓内钉固定中不同的体位牵引方法治疗股骨干骨折的效果 被引量:4

Effect of different body position traction method on closed reduction and intramedullary nail fixation for femoral shaft fracture

摘要 目的对比分析在闭合复位髓内钉固定中不同体位牵引架牵引治疗股骨干骨折的应用效果。方法选取2018年1月至2019年6月四川省康定市人民医院骨科接受闭合复位髓内钉固定术治疗的股骨干骨折患者86例。按数字表法随机分为2两组A组43例行仰卧位牵引架牵引;B组43例行侧卧位牵引架牵引。对比2两组围手术期情况,术后随访6个月。采用Neer评分和Harris评分评价关节功能恢复情况。结果A组术中出血量(226.9±50.3)ml、手术时间(128.4±20.2)min、术中透视时间(201.0±50.3)s、住院时间(14.5±3.5)d、骨折愈合时间(10.4±2.3)个月;B组依次为(123.6±36.2)ml、(110.1±16.8)min、(101.4±35.2)s、(13.1±2.2)d、(8.8±1.7)个月,A组均显著长于B组(P<0.05)。A组复位成功率为81.4%,低于B组的100.0%(P<0.05);A组与B组并发症发生率分别为11.6%、4.7%,组间比较差异无统计学意义(P>0.05)。术后6个月A组的Neer评分及Harris评分优良率均为100.0%;B组均为97.7%,组间比较差异均无统计学意义(P>0.05)。结论在闭合复位髓内钉固定治疗股骨干骨折中应用侧卧位牵引架较仰卧位牵引架具有更高的复位成功率,手术操作更为简便快捷,更有利于术后康复,且骨折愈合时间相对提前。 Objective To compare and analyze the application effect of different body positions traction in the treatment of femoral shaft fracture with closed reduction and intramedullary nail fixation.Methods From January 2018 to June 2019,86 patients with femoral shaft fracture were treated by closed reduction and intramedullary nail fixation in Department of Orthopaedics,Kangding People's Hospital.The cases were divided into two groups according to the random number table:43 cases in group A were treated with supine traction frame;43 cases in group B were treated with lateral traction frame.After 6 months follow-up,Neer score and Harris score were used to evaluate the recovery of joint function.Results The bleeding volume was(226.9±50.3)ml;operation time was(128.4±20.2)min;fluoroscopy time was(201.0±50.3)s;hospitalization time was(14.5±3.5)d;fracture healing time was(10.4±2.3)months in group A.The bleeding volume was(123.6±36.2)ml;operation time was(110.1±16.8)min,fluoroscopy time was(101.4±35.2)s;hospitalization time was(13.1±2.2)d;fracture healing time was(8.8±1.7)months in group B.The recovery times in group A were significantly longer than those in group B(P<0.05).The success rate of reduction in group A was 81.4%,lower than 100.0%in group B(P<0.05);the incidence of complications in group A and group B were 11.63%and 4.65%respectively,there was no significant difference between the two groups(P>0.05).Six months after operation,the excellent and good rates of Neer score and Harris score were 100.0%in group A and 97.7%in group B.There was no significant difference between the two groups(P>0.05).Conclusion In the treatment of femoral shaft fracture with closed reduction and intramedullary nail fixation,the lateral position traction frame has a higher reduction success rate than the supine position traction frame;the operation is more simple and quick.It is more conducive to postoperative rehabilitation.The fracture healing time is relatively early.
